摘  要:目的通过在开颅手术中使用改良筋膜下分离技术,探索一种既能满足手术暴露需要,又不损伤面神经额支,而且能符合美容要求的手术技术。方法选择同期采用传统大骨瓣开颅手术患者(传统组)和改良筋膜下分离技术开颅手术患者(改良组)各35例。改良组采用头皮一颞筋膜一额骨膜瓣整体一层向前翻转直至颅底的方法保护面神经额支。术后第7天评价面神经额支的功能。结果传统组有6例出现面神经额支损伤,改良组无一例出现面神经额支损伤。结论改良筋膜下分离技术能避免面神经额支损伤以致影响容貌,同时达到良好手术视野暴露的目的,值得在临床上推广。Objective To compare the effect of improved subfascial dissection technique with conventional surgical modality in protecting rami frontalis of facial nerve during craniotomy. Methods Seventy patients undergoing craniotomy were randomly divided into two groups: conventional surgery group and modified subfascial dissection group with 35 patients in each group. The function of rami frontalis of facial nerve was evaluated 7d after surgery in both groups. Results Six patients had injury of facial nerve rami frontalis in conventional group, while no one had nerve injury in improved subfascial dissection group. Conclusions The improved subfascial dissection technique can effectively protect rami frontalis of facial nerve from injury during the craniotomy.
