小梁切除术应用生物羊膜联合丝裂霉素C的围手术期护理

摘　　要：目的探讨小梁切除术应用生物羊膜联合丝裂霉素C的围手术期护理体会。方法回顾性分析本院2008年11月～2011年6月收治的27例(27眼)原发性青光眼患者的临床资料,并总结小梁切除术中应用生物羊膜联合丝裂霉素C的围术期护理要点。结果患者术程顺利,术后视力改善4眼(14.8%),视力降低2眼(7.4%),视力不变21眼(77.8%);并发症情况:浅前房4眼,低眼压3眼,均经对症治疗及护理后痊愈出院;术后6～12个月随访,患者的中位眼压为12.3mmHg,较术前明显降低。结论青光眼小梁切除术中联合应用生物羊膜和丝裂霉素C可以减少疤痕的形成,提高滤过功能;而术后针对性的护理能及时发现和处理并发症,提高手术疗效。Objective To summarize the experience in perioperatively nursing patients undergoing trabeculectomy treated by biological amniotic membrane transplantation combined with mitomycin C.Methods The clinical data of 27 patients（27 eyes） with primary glaucoma treated with trabeculectomy in our hospital from November 2008 to June 2011 were analyzed retrospectively to summarize the perioperative care points of biological amniotic membrane transplantation combined with mitomycin C.Results All the patients lived through the operation process smoothly,with visual acuity improved in 4 eyes（14.8%）,growing poorer in 2 eyes（7.4%） and unchanged in 21 eyes（77.8%）.The complications included shallow anterior chamber in 4 eyes and low intraocular pressure in 3 eyes.With the complications managed with symptomatic treatment and nursing,the patients got recovered and discharged.Postoperative 6 to 12 months follow-up showed the average intraocular pressures was（12.3 ± 2.3） mmHg,proving a significant improvement of intraocular pressure compared with pre-trabeculectomy.Conclusion Biological amniotic membrane transplantation combined with mitomycin C during glaucoma trabeculectomy may reduce the scar formation and improve filtration function.The postoperative nursing and timely management of the complications are helpful for the improvement of curative effect.
