摘　　要：目的：探讨地氟醚是否具有诱导心肌预适应作用。方法：ＳＤ 大鼠离体心脏采用Ｌａｎｇｅｎｄｏｒｆｆ 装置灌流。２１只大鼠随机分成对照组（Ｃ组） 、地氟醚组（Ｄ组） 和缺血预处理组（ＩＰ组） ，连续监测ＨＲ，左室内压（ＬＶＰ）及左室内压上升和下降最大速率（ ±ｄｐ／ｄｔｍａｘ）。实验结束时取心肌组织观察其超微结构变化。结果：Ｄ 组和ＩＰ组ＬＶＰ、±ｄｐ／ｄｔｍａｘ 恢复明显优于Ｃ组（ Ｐ＜ ０－０５） ，心肌超微结构损害比Ｃ组轻。Ｄ组和ＩＰ组心功能恢复无统计学差异，心肌结构改变接近。Objective:To investigate whether preconditioning like effects can be induced by desflurame.Methods:Three groups isolated rat hearts( n= 7) were perfused with Langendorff model.The myocardial postischemic reperfusion injury was induced by reperfusion for 30min after stoppage of perfusion for 30min,The hearts were randomly divided into three groups:(1)Control group,no pretreatment before ischemia.(2)Desflurane group,15min of 1MAC desflurane inhalation and 15min of washout.(3)Ischemic preconditioning group; 3 circles of a 5 min ischemia and a 5 min reperfusion.Heart rate (HR),left ventricular pressure(LVP) and its derivation(±dp/dtmax) were monitered.Results:After 30min reperfusion,recovery of HR,LVP,±dp/dtmax of group D and group IP was better than that of group C( P< 0 05).Under the electrionic microscope,there was a relatively lower degree of myocardial ischemia reperfusion damage in group D and group IP as compared with that in group C.The recovery of myocardial function between group D and group IP had no significant difference and myocardial cell damage between group D and group IP was similar.Conclusion:Desflurane pretreatment minics ischemic precconditioning to protect ischemia reperfusion myocardium.
