中心静脉导管胸腔闭式引流与注入抗痨药治疗结核性胸膜炎的疗效观察

摘　　要：目的探讨中心静脉导管胸腔闭式引流术与胸腔注入抗痨药物对结核性胸膜炎的早期治疗意义。方法选取初治结核性胸膜炎患者65例随机分为两组，均予初治化疗方案，治疗组35例行中心静脉导管胸腔闭式引流术并胸腔注入抗痨药物，对照组30例予常规胸腔穿刺术（每周2-3次）。观察两组胸水吸收情况，胸膜增厚粘连及结核中毒症状缓解情况。结果治疗组患者胸水吸收及结核中毒症状缓解快，未发生胸膜明显增厚、分隔及包裹积液，疗效优于对照组。结论中心静脉导管胸腔闭式引流术并胸腔注药治疗结核性胸膜炎安全有效，可推广应用。Objective The discussion about the early treatment of tuberculous pleuritis based on thoracic closed drainage with center venou cather and plus intrapleural injection of anti-tuberalosis. Medthods The 65 enrolled patients with tuberculous pleuritis were randomly divided into therapy and control groups,both of the two groups have the same chemotherapy project,and then The therapy group（n=35）was given horacic closed drainage with center venou cather and plus intrapleural injection of anti-tuberalosis.,the control group（n=30） was given pleural centesis（2 or 3 times a week）. To observe the pleural effusion imbibe, pleural thickening ad- hesion and the poisoning symptoms of tuberculosis relief rate.Results The patients of therapy group have highly relief rate in pleural effusion imbibe and poisoning symptoms of tuberculosis,and there is no pleural thickening adhesion.It is better than control group. Conclusions It is effective and safe that after draining completely the pleural fluid through thoracic closed drainage ,the intracavity injection of anti-tuberalosis.
