摘　　要：目的:探索以区域卫生信息平台为支撑,以居民电子健康档案为基础的新型社区健康管理。方法:以电子健康档案和区域卫生信息平台为基础,以居民周期性体检为依据,分别开展涵盖社区一般人群、高危人群、重点人群的全人群健康管理,并实行动态随访。结果:通过以区域卫生信息平台为支撑,以电子健康档案为基础,以周期性体检为依据的针对全人群健康管理成效显著。结论:社区是实现全人群健康管理的最重要平台,卫生信息化则是其必要的技术支撑。Objective： To explore the new community health management based on the electronic health records of the residents, which is supported by the regional health information platform. Methods： Based on the electronic health record and regional health information platform and the records of the periodic physical examination of residents, we carry out a full covered health management with the dynamic follow-up respectively, which covers all general population in the community as well as the high-risk groups and significant groups. Results： The full covered health management, which is based on the electronic health record and regional health information platform and the records of the periodic physical examination of residents, achieved remarkable effects. Conclusion： The community is the most important platform to achieve the full covered health management, which is essentially supported by the health informatization technically.
