食管钡餐造影在儿童慢性咽炎临床诊断中的应用价值

摘　　要：目的总结食管钡餐造影在小儿慢性咽炎临床诊断中的应用价值,探讨由胃食管反流病(GERD)引起的伴有咳嗽、咽异感等慢性咽炎症状的发病机制。方法选择2008年1月—2010年12月间符合慢性咽炎诊断标准的136例患儿进行临床分析并做食管钡餐造影等一些相关检查,对拟诊为GERD相关性咳嗽、咽异感的17例患儿进行抗反流治疗,并随访观察其疗效。结果本组患儿治疗后症状明显减轻或消失,随访6～12个月,15例病情稳定,未见复发;2例因停药过早导致病情反复,再次用药后症状缓解。治疗期间未发现有何药物不良反应。结论 GERD是引起慢性咳嗽、咽异感的主要病因,因此,对不明原因的慢性咳嗽患儿,应常规做食管钡餐造影的相关检查,对临床疑诊者可给予诊断性治疗,确诊为GERD相关性咳嗽、咽异感的患儿要坚持规范用药,方能取得满意疗效。Objective To summarize application value of esophagography in chronic pharyngitis of children and to investigate the mechanism of cough and foreign body sensation in the pharynx symptoms of chronic pharyngitis caused by gastroesophageal reflux disease（GERD） Methods From January,2008 to December,2010,136 child patients had made clinical analysis and some relevant checkup such as esophag0g- raphy . 17 child patients were diagnosed with GERD- related cough and foreign body sensation in the phar- ynx disease were treated by anti- GERD and were observed curative effect through following up. Results The sick child patients were treated,the symptoms of patients were relieved or disappeared. During a fol- lowing- up of 6 to 12 months, 15 cases with stable disease had not found recurrence and 2cases suffered relqpse after early discontinuation of the drug but the symptoms were alleviated after taking drugs again. Conclusion GERD is the main cause of chronic cough and foreign body sensation in the pharynx. So chil- dren with chronic cough of unknown origin shoud do relevant checkup of esophagography. Dubious patients can give diagnostic treatment. Child patients with GERD- related cough and foreign body sensation in the pharynx disease should use drug sustained and regular.
