摘 要:目的探讨黄葵胶囊联合还原型谷胱甘肽治疗慢性肾脏病(chronic kidney disease,CKD)的临床疗效。方法将60例CKD 2、3期患者随机分为常规组(A组)、常规组+黄葵胶囊组(B组)和常规组+黄葵胶囊+还原型谷胱甘肽组(C组)。A组:优质低蛋白饮食、降压、抗感染等对症治疗。双宫嘧达莫(潘生丁)50 mg,3次/d,福辛普利10 mg,2次/d。B组:在A组治疗基础上加用黄葵胶囊(江苏省苏中制药厂生产)5粒,3次/d,口服。C组:在B组治疗的基础上加用还原型谷胱甘肽120 mg,1次/d,静脉滴注,3组疗程均为2个月。3组患者分别于治疗前后检测24 h尿蛋白、血清清蛋白、血尿素氮、血肌酐、尿N-乙酰-β-氨甘葡萄糖苷酶(NAG)、视黄醇结合蛋白(RBP)及尿β2-微球蛋白(β2-MG)并进行比较。结果治疗后3组患者的24 h尿蛋白水平间差异有统计学意义(P=0.045),且C组的24 h尿蛋白水平明显低于B组,差异有统计学意义(P<0.01),而3组的血清清蛋白、血尿素氮及血肌酐水平间差异均无统计学意义(P>0.05)。治疗后3组患者的尿NAG、BRP、尿β2-MG水平间差异均有统计学意义(P<0.05),且C组与B组比较差异亦均有统计学意义(P<0.05)。结论黄葵胶囊联合还原型谷胱甘肽可减少尿蛋白,明显改善肾小管功能,抑制肾小管纤维化,延缓CKD的进展。Objective To investigate the clinical curative effect of Huangkui capsule combined with reduced glutathi- one in the treatment of patients with chronic kidney disease. Methods 60 patients with chronic kidney disease in CKD2 and CKD3 stages were randomly divided into conventional group ( group A ), conventional treatment plus Huangkui capsule group ( group B) and conventional treatment plus Huangkui capsule as well as reduced glutathione group ( group C) . Group A : diet of low hut quality protein, treatment for reducing blood pressure and anti - infection, dipyridamole 50 mg, three times per day, fosinopril 10 mg, two times per day. Group B: Huangkui capsule (produced by Suzhong pharmaceutical factory in Jiangsu prov- ince) was added to the treatment in addition to conventional treatment with each time 5 capsules, three times per day. Group C: reduced glutathione was added to the treatment in addition to the treatment of group B with each time 120 mg, once per day. Thetreatment course for each group was two months. 24 h urine protein, albumin, urea nitrogen, serum creatinine, urine NAG, RBP and beta 2 - MG were detected and compared before and after the treatment. Results After treatment, the 24 h urine pro- tein of the three groups showed statistically significant difference ( P = 0. 045 ), and the 24 h urine protein of group C was signifi- cantly lower than that of group B ( P 〈 0.01 ) . The levels of albumin, urea nitrogen and serum creatinine in the three groups showed no statistically significant differences ( P 〉 0. 05 ) . After treatment, the levels of urine NAG, BRP and beta 2 - MG showed statistically significant differences (P 〈 0. 05 ), and the group C and group B also showed statistically significant differ- ences (P 〈 0.05). Conclusion Huangkui capsule combined with reduced glutathione can reduce urine protein, improve the function of uriniferous tubules, inhibit the fibrosis of uriniferous tubules and slow down the chronic kidney disease.
