健脾化浊方对糖调节受损的干预作用

摘　　要：目的观察健脾化浊方加减治疗糖调节受损(IGR)患者的临床疗效及其相关机制。方法将90例IGR患者随机分成3组,每组30例。基础组以饮食运动干预,西药组与中药组分别在基础干预的同时采用吡咯列酮片、健脾化浊方加减治疗,疗程均为12周。观察各组治疗前后的体质指数(BMI),血清脂联素、空腹血糖(FBG)、餐后2小时血糖(2hBG)、空腹胰岛素(FINS)、餐后2小时胰岛素(2hINS)、糖化血红蛋白(HbA1c)水平。结果与本组治疗前比较,基础组2hBG、西药组与中药组BMI、FBG、2hBG、HbA1c、FINS、2hFINS均明显降低(P<0.05或P<0.01),脂联素明显升高(P<0.01);与基础组治疗后比较,西药组与中药组各项指标差异均有统计学意义(P<0.05);西药组与中药组治疗后各项指标比较差异均无统计学意义(P>0.05)。结论健脾化浊方具有降低IGR患者血糖水平的作用,其机制可能与升高血清脂联素水平、提高胰岛素敏感性、改善胰岛素抵抗程度有关。Objective To observe the therapeutic effect and mecha nism of Jianpi Huazhuo Formula（decoction for strengthening the spleen and resolving the turbid） for impaired glucose regulation（IGR）.Methods Ninety cases of IGR were randomi zed into 3 groups.The basic treatment group was given diet and exercise interve ntion.Pioglitazone and Jianpi Huazhuo Decoction was added respectively to the w estern medication group and Chinese medication group.The treatment course laste d for 12 weeks.The levels of body mass index（BMI）,serum adiponectin,fasting blood glucose（FBG）,postprandial 2 hours blood glucose（2hBG）,fasting insulin（FINS）,postprandial 2 hou rs insulin（2hINS） and glycated hemoglobin A1c（HbA1c） were observed.Res ults After treat ment,the level of 2hBG was significantly decreased in the basic treatment group（P0.05 or P0.01）.The levels of BMI,FBG,2hBG,HbA1c,FINS and 2hINS were significantly decreased in the western medication group and Chinese medication group（P 0.05）,but the level of serum adiponectin increased significantly（P0.01）.Com parin g with the basic group,the index mentioned above obviously improved in the west ern medication group and Chinese medication group（P0.05）.There was no si gnificant difference between the western medication group and Chinese medication grou p（P0.05）.Conclusion Jianpi Huazhuo Formula co uld regulate the level of bloo d glucose,which maybe relevant to elevating the level of serum adiponectin,inc reasing insulin sensitivity and improving insulin resistance,
