早产儿暂时性低甲状腺素血症的临床观察

摘　　要：目的为早产儿THOP的早期合理干预、激素替代治疗提供实验数据和理论依据。方法选取2009年1月～2012年1月我院收治的早产儿270例，其中确诊为THOP90例，并将该90例THOP患儿随机分为观察组和对照组各45例，观察组45例予常规治疗基础上加用优甲乐治疗，对照组45例患儿予常规治疗。对比观察组和对照组患儿的血T3、T4、TSH变化。同时对THOP组、正常早产儿组患儿T4、并发症、黄疸消退时间、住院时间等进行比较分析。结果THOP组患儿T4水平明显低于正常早产儿组，且T4水平越低，黄疸消退时间越长，自行排便次数少，住院期间体重增长速率慢，住院时间长，差异有统计学意义（P〈0．05或P〈0．01）。观察组应用优甲乐治疗后患儿T3、T4、TSH较治疗前及对照组显著提高，差异有统计学意义（P〈0．05）。结论早产儿暂时性低甲状腺素血症患儿T4水平较低，黄疸消退时间长，自行排便次数少，住院时间长，经给予优甲乐治疗后上述症状明显缓解，T3、T4、TSH水平明显升高。Objective To provide experimental data and theoretical basis for early preterm children THOP reasonable in- tervention. Methods All 270 cases preterm children admitted to our hospital were confirmed THOP 90 cases from Jan- uary 2009 to January 2012, 90 cases THOP were randomly divided into two groups and the control group 45 cases, 45 cases observed for euthyrox treatment, 45 cases children in the control group were given conventional treatment. The T3, T4, TSH changes and their children through the skin and total bilirubin were compared. At the same time T4 of children with THOP was lower than the control group, complications, jaundice time, length of stay were compared. Results THOP group of children with normal T4 levels were significantly higher than the group of premature children, jaundice for a long time, fewer bowel movements on their own, slower rate of weight gain during hospitalization, long hospital stay, the difference was significant （P 〈 0.05 or P 〈 0.01）. application euthyrox observation group after treatment T3, T4, TSH before treatment and the control group significantly increased, the difference was significant （P 〈 0.05）. Conclusion The transient hypothyroxine- mia in preterm children with lower levels of T4 in children, jaundice for a long time, fewer bowel movements on their own, long hospital stay, by giving euthyrox the symptoms were relieved after treatment, T3, T4, TSH rose significantly high.
