替米沙坦对心房颤动患者心房纤维化及血清apelin-12,PⅢP水平的影响

摘　　要：目的 观察替米沙坦对心房颤动（Af）患者心房纤维化及血清apelin-12、血清Ⅲ型前胶原末端肽（PⅢP）的影响并探讨其可能的作用机制及apelin-12与心房纤维化的关系.方法 选择76例Af患者,随机分为常规治疗组和替米沙坦组,应用超声心动图记录35例健康体检者（正常对照组）及2组Af患者治疗前及治疗3,6,12个月后左房内径（LAD）（前后径）,同时应用酶联吸附法测定血清apelin-12,PⅢP浓度.结果 Af患者较正常对照组LAD,PⅢP增高,apelin-12降低.治疗3个月后替米沙坦组较常规治疗组apelin-12增高,PⅢP降低（P；＜0.05）；治疗6个月后常规治疗组LAD明显大于替米沙坦组（P＜0.01）；Pearson直线相关分析发现LAD与PⅢp呈正相关（P＜0.01）；apelin-12与PⅢP,LAD呈负相关（P＜0.01）.结论 替米沙坦能够升高血清apelin-12,降低血清PⅢP的浓度,减轻心房纤维化,抑制左心房扩大,血清ape lin-12或许可作为反映心房纤维化的指标.Objective To study the relationship hetween apelin-I 2 and atrial fibrosis anti explore the effects of telmisartan on At- rial fibrosis and on the level of serum apelin-12 and PIIIP in patients with atrial fibrillation（ Af）. Methods The 76 patienls with Af were randomly divided into the routine therapy group and the telmisartan group. The left atrial diameter（LAD） （ anteroposterior diameter） was measured in 35 cases of heahhy persons（ normal control group ） and the two groups of Af patients before and at the 3rd ,6rd, 12 rd month after treatment hy doppler uhrasound,meanwhile, the level of serurn apelin-12 and procollhtgen III （P III P） were measured by enzyme linked immunosorhent assay. Results Two groups of patients with Af LAD, PIIIP increased,apelin-12 decreased. From the third months after treatment in telmisartan group compared with tile routine therapy group and apelin-12 increased, PIII P decreased （P 〈 0.05 ） ;The routine therapy group I.AI） was significantly greater than that in telmisartan group after 6 months treatment （ P 〈 0. 01 ）. Pearson linear correlation analysis showed that LAD and PIIIP was positively correlated（ P 〈0.01 ） ;apelin-12 and PIll P, 1,AD way negatively correlated （P 〈0.01）. Conclusions Tehnisartan elevates serum apelin-12,reduces the concentration of serum P Ill P and reduces Atrial fihrosis,inhibites the expansion of the left atrium,serum apelin-12 may serve as an indicator refleetes the Atrial fibrosis.
