腰椎退变性侧凸畸形合并骨质疏松28例术后2年随访结果分析

摘　　要：目的探讨腰椎退变性侧凸合并骨质疏松患者的手术治疗效果及其术后影响因素。方法回顾分析2007年12月～2009年12月收治的28例腰椎退变性侧凸合并骨质疏松患者的临床资料。所有患者均获得2年以上随访。ODI、VAS以及患者临床特点在术前和末次随访时进行评价。评价功能恢复和影像学参数之间的关系。结果术后随访结果显示VAS平均评分由术前的(8.3±1.4)分改善为末次随访的(3.6±1.1)分;ODI平均评分由术前的(56.26±9.13)分改善为末次随访的(34.13±9.15)分。患者满意度为75%。术后影像学参数包括Cobb角和椎体稳定与ODI评分满意度相关联。结论本研究表明腰椎退变性侧凸畸形合并骨质疏松患者可以通过强化椎弓根螺钉固定及椎体间和后路融合进行治疗,临床效果满意。Objective To investigate the surgical treatment effect and its impact factors after operation of degeneratie lumbar protrusion deformity side with osteoporosis.Methods Retrospective analyzed was conducted on 28 consecutive patients with degenerative lumbar scoliosis combined with osteoporosis who had surgery from December 2007 to December 2009.All had a 2-years follow-up.Oswestry disability index（ODI）,visual analog scale（VAS）,and clinical characteristics were evaluated before surgery and last follow-up period.The relationship between the difference of radiographic parameter and functional outcome was evaluated.Results Postoperative follow-up results showed that the average score VAS by preoperative（8.3±1.4） points for the end time improve follow-up for（3.6±1.1） points;ODI average score by preoperative（56.26±9.13） points for the end time improve follow-up for（34.13±9.15） points.Patient satisfaction was 75%.Postoperative radiographic parameters including Cobb Angle and vertebral stability and ODI satisfaction scores related.Conclusion This research shows that degeneratie lumbar protrusion deformity side with osteoporosis patients can by strengthening the pedicle screws and posterior fusion between vertebral bodies and treatment,the clinical with satisfactory effect.
