摘  要:目的研究CD257/CD256及其受体在发生排斥反应移植肾组织中的作用。方法收集1987年1月至2007年7月间因移植肾失功切除的移植肾组织标本46份和2009年行程序性活组织检查的移植肾组织标本10份。运用免疫组织化学法分别检测了CD256、CD257、BAFF-R、BCMA、TACI及CD138、CD3和CD20等分子的表达。采用图像分析软件Histo Quest Analysis和统计分析软件SPSS16.0对表达结果进行统计分析。结果依据Banff 2005标准,对移植肾组织标本进行病理学分组:抗体介导的排斥反应(ABMR)组15例、T细胞介导的排斥反应(TCMR)组16例和未明原因病例(UAC)组15例。免疫组织化学结果显示,CD256、CD257及其受体BAFF-R、BCMA和TACI均表达于ABMR组和TCMR组肾小管上皮细胞的胞膜或胞浆,而UAC组和正常对照组无表达或弱表达。CD257在ABMR组、CD256在ABMR组和TCMR组的表达与其他组比较差异有统计学意义(P<0.05)。BAFF-R和BCMA在ABMR组的表达与其他组比较差异有统计学意义(P<0.05);而TACI在ABMR组和TCMR组的表达与UAC组和正常对照组比较差异有统计学意义(P<0.05)。CD138+细胞除浸润于肾组织间质外,也浸润于肾小管上皮细胞胞浆或胞膜。CD138+细胞在ABMR组的表达水平高于其他组(P<0.05)。CD3+细胞主要浸润于肾组织间质,以散在分布为主,在ABMR组和TCMR组均有表达,但在TCMR组表达水平更高。CD20+细胞亦主要分布于肾组织间质,成簇分布为主,在ABMR组和TCMR组的表达水平相似。相关性分析显示,CD256与BCMA、TACI和CD138+细胞具有显著相关性;而CD257与BAFF-R、BCMA、TACI和CD138+细胞均具有显著相关性。结论 CD256信号与CD257信号可能均参与了ABMR进程。Objective To investigate the potential role of CD257 and CD256 signals in progression of renal allograft rejection.Methods From January 1987 to July 2007,allografts from 46 renal recipients were excised due to dysfunction and the tissues were collected.Other 10 tissue samples from protocol biopsies of renal allografts in 2009 were selected.Immunohistochemistry staining was applied to detect molecules,such as CD256,CD257,BAFF-R,BCMA,TACI,CD138,CD3 and CD20.HistoQuest Analysis software was used to read and analyze the staining results,and SPSS16.0 was applied to statistical analysis.Results Pathological diagnosis was made according to Banff 2005 guidelines: antibody-mediated rejection(ABMR) in 15 cases,T-cell-mediated rejection(TCMR) in 16 cases,and unknown aetiology cases(UAC) in 15 cases.CD256,CD257,and their receptors BAFF-R,BCMA,and TACI were expressed on the membrane or cytoplasm of renal tubular epithelial cells in the ABMR and TCMR groups,while no or weak expression of them was observed in the UAC group and in protocol biopsies.Strong staining of CD257 was noted in the ABMR tissues,and strong staining of CD256 was noted both in the BCMR and TCMR tissues,and as compared with other groups,statistical significance was noted(P0.05).The receptors BAFF-R,BCMA and TACI were all strongly stained in the ABMR tissues,and TACI strongly stained in the TCMR tissues,and compared with other groups,statistical significance was found(P0.05).CD138 molecule was observed in the renal interstitium and the membrane and cytoplasm of renal tubular epithelial cells.The mean expression of CD138+ cells in the ABMR tissues was statistically higher than that in the other groups(P0.05).CD3 molecule mainly scattered in the renal interstitium in the ABMR and TCMR tissues,and more seen in the TCMR tissues.CD20 molecule always clustered in the renal interstitium,and had a similar expression level in the ABMR and TCMR groups.The correlation analysis indicated that CD256 was significantly correlated with BCMA,TACI,a
