摘  要:目的:应用应变率成像(SRI)检测无微血管病变的2型糖尿病(DM)患者和健康人的局部心功能,探讨SRI的优越性。并希望为临床提供对糖尿病需早期、长期干预的有力依据。方法:对临床诊断无微血管病变的2型糖尿病患者和健康人群,应用常规超声心动图及SRI对左室整体收缩和舒张功能及室壁各节段的局部收缩和舒张功能进行测值、对比分析。结果:常规超声检测左室整体收缩及舒张功能,两组间无差异。SRI检测局部收缩和舒张功能,DM组左室侧壁基底段与后间隔基底段收缩功能减低,P<0.05;DM组左室侧壁基底段、后间隔基底段及下壁中段的舒张早期平均峰值应变率(SRe)减低,P<0.05。结论:SRI可以准确评估无微血管病变的2型DM患者在整体心功能正常时局部心肌功能的损害,这为临床提供了对糖尿病需早期干预、治疗的有力证据。Objective:With strain rate imaging(SRI) to detect the local cardiac function of patients with 2 diabetes without microangiopathy and the health.The advantage of SRI was explored.A strong basis was hoped to be provided for clinical that the diabetes need early,long-term treatment.Methods: None of the clinical diagnosis of microvascular disease in type 2 diabetic patients and healthy people,using the conventional ultrasonic and SRI to detect and compare the overall systolic and diastolic function and the regional systolic and diastolic function.Result: The left ventricular overall systolic and diastolic function is detected by conventional ultrasonic.The comparative result of two groups is no statistical significance.The regional systolic and diastolic function is detected by SRI.The systolic function of the basal inferoseptal and the basal lateral of the case group is reduced,P0.05.The early diastolic peak strain rate(SRe) of the basal inferoseptal、the basal lateral and the middle inferior of the case group is reduced,P0.05.Conclusion: The damaged regional myocardial function of patients with type 2 diabetes without microangiopathy can be accurately assessed by SRI when the overall myocardial function is normal.Strong evidence is provided that the diabetes needs early intervention,treatment for clinical.
