不同促性腺激素促排卵对IVF-ET结局的影响

摘　　要：目的比较基因重组卵泡刺激素(r-FSH)及尿促卵泡刺激素(u-FSH)对体外受精-胚胎移植(IVF-ET)结局的影响。方法回顾性分析IVF-ET黄体中期长方案降调超促排卵173个周期的临床资料,其中应用r-FSH 69个周期,u-FSH 104个周期。比较两组促性腺激素(Gn)用量、药物费用、应用天数,注射人绒毛膜促性腺激素(HCG)日雌二醇(E2)、黄体生成素(LH)、孕激素(P)水平,以及≥14mm卵泡数、获卵数、优胚数、着床率、妊娠率及流产率等。结果 r-FSH组Gn用量显著少于u-FSH组(t=-4.017,P<0.001),r-FSH组Gn费用明显高于u-FSH组(t=9.282,P<0.001),但Gn应用天数两组间差异无显著性(t=0.920,P>0.05)。两组注射HCG日E2、LH及P水平差异无显著性(t=-0.539～0.938,P>0.05);两组注射HCG日≥14mm卵泡数、获卵数、优胚数比较差异无显著性(t=-0.255～1.778,P>0.05),着床率、妊娠率及流产率比较差异亦均无显著意义(χ2=0.138～3.142,P>0.05)。结论 r-FSH与u-FSH用于IVF-ET临床治疗结局无差异,u-FSH存在明显的价格优势。Objective To compare the outcome between recombinant follicle stimulating hormone（r-FSH） and urinary follicle stimulating hormone（u-FSH） for in vitro fertilization and embryo transfer（IVF-ET） cycles. Methods A retrospective analysis was done for medical records of 173 patients who had undergone IVF-ET treatment following down regulation with a long midluteal phase protocol were divided into r-FSH group（69 cases） and u-FSH group（104 cases）.The outcomes of the two groups were compared. Results The dosages of Gn used in r-FSH group were significantly lower than that in u-FSH group（t=-4.017,P〈0.001）,but the cost was higher（t=9.282,P〈0.001）.The difference of the days of Gn stimulation between the two groups was not significant（t=-0.920,P〈0.05）.Serum estradiol（E_2）,luteinizing hormone（LH） and progesterone（P） levels on HCG day were not significantly different（t=-0.539-0.938,P〈0.05）.There was no significant difference in the number of follicle ≥ 14 mm on HCG day,oocytes retrieved,as well as fine quality embryo,（t=-0.255-1.778,P〈0.05）.There was also no significant difference between the two groups in terms of nidation,pregnancy rate and abortion rate（χ2=0.138-3.142,P〈0.05）. Conclusion The treatment outcome of using r-FSH or u-FSH for IVF-ET is similar,but the cost of u-FSH is dramatically lower.
