食管心房调搏检查在116例室上速射频消融术中的应用分析

摘　　要：目的:评价食管心房调搏(TEAP)检查在室上速行射频消融术中的价值意义。方法:选取初步诊断为阵发性室上性心动过速(PSVT)且在做射频消融术(RFCA)之前均先行经食管心房调搏(TEAP)检查的患者116例,对比观察TEAP时多导ECG检查资料与心内电生理资料。结果:本组116例初诊为PSVT患者、其中110例TEAP诊断与心内电生理及射频消融结果相符合,TEAP与心内电生理符合率为94.8%,6例TEAP诊断与心内电生理及射频消融结果不符合,其诊断错误主要由于P波记录不清、RP标测不准确、属于复杂病例及不能观察室房逆行传导等局限性造成。结论:在室上速行射频消融术之前行TEAP电生理检查可初步了解PSVT性质、分类、简化射频消融操作程序、缩短手术时间、降低放射线照射损害等优点,值得在临床推广应用。Objective：To evaluate the diagnostic value of transesophageal artial pacing（TEAP） before Radiofreqency catheter ablation（RFCA） in patient with paroxysmal supraventricular tachycardia（PSVT）.Method：116 patient with PSVT underwent transesophageal elec trophysiological examination before RFCA,which was compared with endocardial electrophysiological examination data.Results：The diagnostic result of TEAP was consistent with that of endocardial electrophysiological examination in 110 patient（94.2%） but the other was not.The reason of misdiagnose in other 6 patients were unclear recording of P wave,inaccurate measurement of RPE interval and unable to observe ventriculoatrial（VA） conduction.Conclusion：These results suggest performing TEAP before RFCA in patient with PSVT can find out the character and classification of PSVT initially,smiply the process of RFCA,shorting operating time and reduce exposure time and damage of X-ray.This method can be applied in the clinical practice.
