车载运动状态下便携式连续性血液净化机的操作方法及技巧

摘　　要：目的探讨在救护车转运伤员行驶途中实施连续性肾脏替代治疗（continousrenalreplacementtreatment，cRRT）时，便携式连续性血液净化机（mobilecontinousbloodpurificationmachine，MCBPM）的操作方法和技巧。方法选择健康犬在行驶的救护车上进行CRRT，观察试验犬生命体征，妥善摆放置换液及血液管路，体验CRRT及机器操作的最佳方法。结果机器采用四角气压吸盘固定牢靠；在救护车狭小空间中，护士以“蹲姿倚靠”或“坐位操作”为实施CRRT的最佳方式；前臂支撑抽取或配制药液，严禁注射器覆帽动作，防针刺伤；悬挂牢靠或低置袋装液体，防止滚落。结论车载动物试验证明：护士经过特殊培训，完全能够在救护车行驶过程中，准确的操作便携式连续性血液净化机，不间断地实施紧急血液净化治疗。Objective To study the operation methods and skills of operating the mobile continuous blood purification machine（MCBPM） on ambulance while carrying out continuous blood purification treatment for the injured during transportation. Methods To choose healthy dogs for CRRT therapy on a moving ambulance, observe vital signs of dogs, properly place replacement fluid and blood pipes, experience the best operation way of MCBPM. Results The MCBPM was fixed firmly on the floor with the four-comer pressure suction; ＂Squatting and leaning against the wails of the cabin＂ or ＂seating on bench＂ was the best way for the operation of MCBPM in small space on the ambulance; Prop forearm while extracting or preparing of liquid, no covering syringe cap to prevent needle stick injuries; Firmly hang or low-set hang the bag of liquid to prevent rolling. Conclusion The experiments on ambulance proved that nurses could properly operate the MCBPM on a moving ambulance after specially trained, which would bring an uninterrupted emergent blood purification treatment.
