摘　　要：目的探讨2μm激光治疗老年前列腺增生症(BPH)的疗效和安全性。方法收集2010年1月～2011年8月应用2μm激光治疗老年BPH156例,进行临床疗效分析。结果 156例患者手术均成功,平均手术时间为(110±9)min,术中平均出血量为(73±8)ml,术中未输血、无电切综合征发生。平均住院时间(5.00±0.45)d,术后自主排尿通畅;无尿道狭窄;5例轻度尿失禁患者经锻炼后,排尿功能恢复;2例腺体组织形成活瓣阻挡排尿,经膀胱镜下取出残余腐肉后,排尿通畅。手术前后各项指标变化比较,差异有统计学意义(P<0.05)。结论 2μm激光手术系统治疗老年BPH安全有效,术中、术后出血量少,并发症少,住院时间短,术后患者排尿功能及生活质量有明显改善。Objective to explore the safety and clinical efficacy of 2 microm continuous-wave laser vaporesection for the treatment of benign prostatic hyperplasia.in elderly patient.Methods From Jan,2010 to Aug.2011,we treated 156 patients with benign prostatic hyperplasia using a 70w 2 microm continuous-wave laser and the operative outcomes assessed were resection time,bleeding volume,the mean hospitalization.The following variables were analyzed before and after vaporesection：the serum level of prostatic specific antigen,maximum urinary flow rate（Qmax）,postvoiding residual urine volume,international prostate symptom score,quality of life.Results all the operations are succeeded.The mean vaporesection time was（110±9） min,the mean bleed volume was（73±8） ml,the mean hospitalized time was（5.00±0.45） d.no patient required blood transfusion,no case of transurethral resection syndrome was observed,all patients were satisfied with voiding outcome after vaporesection.No case developed urethral stricture,5 cases with mild urinary incontinence recovered after exercise.Conclusions vaporesection using the 2-microm continuous-wave laser for the treatment of BPH is a safe and effective ablative procedure with minimal complications and rare bleeding.
