湖北汉族人群对乙肝疫苗免疫应答能力与HLA-DRB1等位基因相关性的研究

摘　　要：目的分析HLA-DRB1等位基因型别与个体乙肝疫苗免疫应答水平的相关性。方法对37名湖北汉族健康自愿者进行HBV血源型疫苗标准全程接种,共3次(0,1,6月),末次接种后8周用酶免疫法(EIA)检测血清抗-HBs抗体水平(S/N≥2.1为应答者,S/N<2.1为无应答者);同时,对全部受试者进行HLA-DRB1基因分型。结果37名个体中无应答者7名(19%),应答者30名(81%),无应答与HLA-DRB*1001等位基因具有显著相关性,RR=21.75,X2=5.55,P<0.05。结论在湖北汉族人群中。ObjectiveThe aim of the present work is to study the association between HAL DRB1 alleles and the feature of the immune response to hepatitis B vaccine among different individuals.Methods37 health volunteers from Hubei of the Han nationality were subjected to a standard course of immunization with HBV plasma vaccine at month 0,1 and 6,successively,8 weeks after the last immunization,the serum level of anti HBs antibody was examined with EIA.Individuals with S/N≥2.1 were rated as responders,while those with S/N<2.1 were nonresponders.Results30(81%)of the 37 subjects were responders,and only 7(19%) were nonresponders.Unrespon siveness of these 7 individuals to hepatitis B vaccine were found to be closely associated with HLA DRB 1001 allele,(RR=21.75, X 2=5.55, P <0.01),as assayed with the PCR SSP typing.ConclusionThe level of immune response to hepatitis B vaccine of individual bearing HLA DRB 1001 allele is significantly lower than others.
