摘  要:目的 探讨右美托咪定对体外循环心内直视手术患者中性粒细胞黏附分子表达的影响.方法 选择20例主动脉阻断时间30min以上体外循环转机60min以上的心内直视手术患者,随机分成实验组(DEX组)和对照组(C组).DEX组患者于入手术室后给予右美托咪定1μg/kg负荷量(15min输注完毕),然后以0.7μg/(kg·h)持续静脉注射.C组不用右美托咪定.分别在桡动脉穿刺成功后(T1)、麻醉诱导后15min(T2)、转机后30min(T3)、主动脉开放30min(T4)用流式细胞仪测定中性粒细胞CD11b的活性表达.结果 两组患者T1,T2,T4各时点中性粒细胞CD11b水平差异无显著性(P〉0.05),但T3时DEX组CD11b表达低于C组(P〈0.05).结论 右美托咪定能抑制心内直视手术患者中性粒细胞黏附分子CD11b的表达.Objective To investigate the effect of dexmedetomidine on the expression of adhesion molecules on polymorphonu- clear neutrophil in patients undergoing open heart surgery with extracorporeal circulation. Methods Twenty patients scheduled for cardiac surgery beyond 30rain following eardiopulmonary bypass( CPB) and 60min following extracorporeal circulation,were randomly divided into two groups, test group ( group DEX) and control group ( group C ), in test group ( group DEX ) dexmedetomidine 1 μg/kg was administered intrave- nously before anesthesia induction and 0.7μg/( kg · h) continuously, but in control group(group C), no dexmedetomidine was given. The concentration of neutrophi CD11 b by flow cytometry was measured after radial artery puncture ( T1 ), 15min after anesthesia induction( T2 ) and 30rain of CPB ( T3 ) and 30rain after CPB. Results The levels of CD11 b showed no difference between groups after radial artery puncture at T1 ,T2 and T4 ,while in group DEX,the level of CD11 b was obvious reduced at T3 compared to group C. Conclusion Dexmedetomidine can reduce the level of CD1 l b in patients undergoing open heart surgery with extraeorporeal circulation.
