摘  要:目的:观察不同浓度LPSp对HBV宫内感染孕妇的脐血单个核细胞分泌IL-4、IFN-γ及细胞表面TLR4表达的影响。方法:选择外周血HBsAg、HBeAg、HBcAb阳性且脐血HBV-DNA阳性孕妇25例为试验组,根据加入LPSp的不同分为:无LPSp组、LPSp 1μg/L组、LPSp 10μg/L组、LPSp 100μg/L组;以外周血HBV标志物为阴性的正常分娩孕妇20例为对照组。分离CBMC并培养,采用ELISA法检测培养上清液中的IFN-γ、IL-4的浓度,用流式细胞仪检测CBMC表面TLR4表达。结果:①LPSp各组中IFN-γ、IL-4水平与无LPSp组比较差异均有统计学意义(P<0.01),IFN-γ水平与LPSp浓度呈正相关(r=0.778),IL-4水平与LPSp浓度呈负相关(r=-0.927)。②TLR4表达与IFN-γ水平呈正相关(P<0.05),TLR4与IL-4呈负相关(P<0.05);TLR4表达水平与LPSp浓度呈正相关(r=0.754)。结论:不同浓度的LPSp可能通过影响TLR4信号传导通路诱导CBMC分泌不同细胞因子,选择性调控Th1/Th2型应答。Objective:To observe the effects of different concentrations of pantoea agglomerans lipopolysaccharid(LPSp) on the secretions of IL-4 and IFN-γ by mononuclear cells and TLR4 expression on the surface of mononuclear cells in umbilical cord blood of pregnant women with HBV intrauterine infection. Methods:Twenty-five pregnant women with positive HBsAg,HBeAg,and HBcAb in peripheral blood and positive HBV-DNA in umbilical cord blood were selected as study group,then they were divided into non-LPSp group,1 μg/L LPSp group,10 μg/L LPSp group,and 100 μg/L LPSp group according to the concentration of LPSp;twenty normal pregnant women with negative HBV markers in peripheral blood were selected as control group.CBMC were isolated and cultured,ELISA was used to detect the concentrations of IFN-γ and IL-4 in liquid supernatant,the expression of TLR4 on the surface of CBMC was detected by flow cytometry. Results:There was significant difference in the levels of IFN-γ and IL-4 between non-LPSp group and the other three groups(P0.01),there was a positive correlation between IFN-γ level and LPSp concentration(γ=0.778),while there was a negative correlation between IL-4 level and LPSp concentration(γ=-0.927).There was a positive correlation between TLR4 expression and IFN-γ level(P0.05).There was a negative correlation between TLR4 expression and IL-4 level(P0.05).There was a positive correlation between TLR4 expression and LPSp concentration(γ=0.754). Conclusion:Different concentrations of LPSp can induce CBMC to secret different cytokines by affecting TLR4 signaling transduction pathway,which can regulate Th1/Th2 response.
