摘  要:目的:观察冠状动脉慢血流(SCF)与血清脂联素(ANP)、高敏C反应蛋白(hs-CRP)水平之间的相关性。方法:选择行冠状动脉造影(CAG)显示冠状动脉无明显病变的患者70例,通过校正TIMI血流计帧法(cTFC)计算各支冠状动脉的血流帧数。冠状动脉血流平均帧数≥27定义为SCF,将患者分为SCF组(35例)和对照组(35例)。检测ANP和hs-CRP水平。结果:SCF组血清的APN水平低于对照组[(4.88±0.34)μg/ml∶(7.34±0.48)μg/ml,P<0.05],而其血清hs-CRP水平高于对照组[(2.13±1.07)μg/ml∶(1.29±0.69)μg/ml,P<0.05],Logistic回归分析表明hs-CRP是影响SCF的危险因素(OR=2.44,P=0.04),APN是SCF的保护性因素(OR=0.94,P=0.03)。结论:血清hs-CRP水平升高、APN水平降低与SCF相关,炎症反应在SCF发生的病理生理过程中起重要作用。Objective:To study the relationship between serum levels of adiponectin(APN),high-sensitivity C-reactive protein(hs-CRP) and slow coronary flow(SCF).Method:The study population included 70 patients with angiographically proven normal coronary artery.Coronary flow patterns of the cases were determined by the corrected thrombolysis in myocardial infarction frame count(cTFC) method.The cTFC was calculated separately for each coronary artery and their average was determined as the mean cTFC for each subject.Patients whose average cTFC more than 27 were included SCF group(n=35) and the others were included the control group(n=35).Serum APN and hs-CRP were detected.Result:The level ofserum APN were lower in the SCF group compared with the control group [(4.88±0.34)μg/ml∶(7.34±0.48)μg/ml,P0.05],whereas the level of serum hs-CRP were higher in the SCF group than the control group [(2.13±1.07)μg/ml∶(1.29±0.69)μg/ml,P0.05].Logistic regression analysis showed that the serum hs-CRP was a risk factor of SCF(OR=2.44,P=0.04),and the serum APN was a protective factor(OR=0.94,P=0.03).Conclusion:The higher level of serum hs-CRP and the lower level of serum APN are associated with the SCF.Inflammation may play an important role in the pathogenesis of the SCF phenomenon.
