医院药师参与临床路径工作的体会

摘　　要：目的:探讨医院药师在临床路径实施过程中的作用。方法:采用实证案例研究方法,对我院神经内科实施临床路径中医院药师的工作模式进行分析。结果:医院药师可在临床药师的帮助和指导下,学习临床路径相关知识;可在保证临床路径药品正常供应的同时,抓住临床路径相关药学监护点进行药学监护。结论:在临床药师短缺的情况下,医院药师通过学习及实践,利用病房药房的工作平台做好各阶段临床路径的药学监护,可确保患者用药的合理性及临床路径顺利实施。OBJECTIVE： To investigate the role of hospital pharmacists in the implementation of clinical pathway. METHODS： The working mode of hospital pharmacists in the implementation of clinical pathway in neurology department of our hospital was analyzed by using empirical case study. RESULTS： Hospital pharmacists should learning knowledge of clinical pathways under the help of clinical pharmacists; at the same times, to ensure regular drug supply of clinical pathways, they should seize the clinical pathway-related pharmaceutical care point to provide pharmaceutical care. CONCLUSION： In the case of a shortage of clinical pharmacists,hospital pharmacists provide patient-centered pharmaceutical care in every stage of clinical pathway through the platform ofinpatient pharmacy,thus ensure rational drug use of patient and smooth implementation of clinical pathway through study and practice.
