摘　　要：目的:加强荆门地区医院外科围术期抗菌药物预防性应用与管理。方法:回顾性整群抽取荆门地区19家医院2010、2011年4-9月Ⅰ类切口手术(甲状腺、乳腺、腹外疝)患者病历3027份,将2010年4-9月1489份手术病历设为干预前组(A组),2011年4-9月1538份手术病历设为干预后组(B组),对2组抗菌药物使用合理性与平均住院时间、费用等进行统计、分析。结果:与A组比较,B组抗菌药物使用率降低、病原学送检率增加、抗菌药物种类选择合理性提高、给药时机合理率显著增加、平均用药疗程明显缩短、单一用药比例大幅提高、平均住院时间减少、人均住院总费用降低(P<0.05);2组术后感染率比较差异无统计学意义(P>0.05)。结论:干预后,该地区医院Ⅰ类切口手术围术期预防用抗菌药物趋于科学、合理、规范,医疗成本显著降低。OBJECTIVE： To strengthen the perioperative prophylactic application and management of antibiotics in hospitals from Jinmen area. METHODS： 3 027 medical records of typeⅠincision surgery （thyroid, breast, abdominal external hernia） were collected from 19 hospitals in Jinmen area during Apr.-Sept. in 2010 and 2011 retrospectively. 1 489 medical records during Apr.-Sept. 2010 were included in before intervention group （group A）, and 1 538 medical records during Apr.-Sept. 2011 were included in after intervention group （group B）. The reasonability of antibiotics use, average hospitalization stay and cost were analyzed statistically in 2 groups. RESULTS： Compared with group A, the utilization ratio of antibiotics in group B decreased, the delivered rate of the etiology specimens, the reasonability of type selection of antibiotics, reasonable rate of medication time and proportion of single drug increased significantly, but average treatment course, average hospitalization stay and hospitalization cost per capita decreased （P0.05）. There was no statistical significance in post-operation infection rate between 2 groups （P0.05）. CONCLUSION： After intervention, the perioperative prophylactic application of antibiotics in typeⅠincision surgery from the hospitals of this area tends to be scientific, reasonable and standard, and medical cost decreases significantly.
