以检验报告质量为指标确定室内质控的分析批长度

摘　　要：目的以检验报告质量为指标确定室内质量控制的分析批长度，使得既保证检验质量，又降低质控成本。方法以不合格检验结果个数≤1为目标，采用概率计算的方法，预测不同分析性能的项目在出现不同类型误差（间断误差和持续误差）时，分别应采取的分析批长度作为推荐分析批长度，并将此与实际分析批长度做比较。结果所选项目分析性能介于2．83～6．01Sigma，存在间断误差时的推荐分析批长度介于942～45737062445个测试数，存在持续误差时的推荐分析批长度介于480-43907579947个测试数。分析性能≥3Sigma时，推荐分析批长度远远长于实际分析批；分析性能〈3Sigma时，推荐分析批长度接近甚至短于实际分析批。结论分析性能≥3Sigma的项目可根据实际情况适当延长分析批长度，降低质控成本；分析性能〈3Sigma的项目需小心谨慎，必要时可根据推荐分析批长度，缩短质控时间间隔，保证检验质量。Objective To illustrate a method which design analytic run length focusing on quality of reported patient results. Methods Intermittent and persistent errors were studied. The recommended analytic run length which guaranteed the expected increase in the number of unacceptable patient results reported below one was summarized by statistical for varied analytic performance. Results The analytic performance was between 2. 83 - 6.01Sigma. Their recommended analytic run length was between 942-45 737 062 445 tests when intermittent error appeared,while it was between 480-43 907 579 947 tests when persistent error appeared. When analytic performance was above 3Sigma, the recommended analytic run length was much longer than that of actual,while it was below 3Sigma, the recommended analytical run length was near, even shorter than that of actual. Conclusion If analytic performance is above 3Sigma,the actual analytic run length should be extend- ed to reduce cost,while it is below 3Sigma,the actual analytic run length might be shorten to guarantee quality.
