小组包干制整体护理对提高母乳喂养质量的效果研究

摘　　要：目的:评价产科病区护士分层管理下小组包干制整体护理对提高母乳喂养质量的效果。方法:将产科病区护士分三层,即责任组长-高级责任护士-初级责任护士,实行小组包干制整体护理,明确各层护士在促进母乳喂养中的职责。比较实施前后产科病区产后2天以上的产妇乳房胀痛、乳头皲裂、乳头错觉发生率,出院时婴儿纯母乳喂养率。结果:实施前产妇乳房胀痛、乳头皲裂、乳头错觉、纯母乳喂养率,分别为27.8%、35.5%、3.7%和68.7%,实施后分别为11.8%、16.2%、1.5%和85.2%,除乳头错觉发生率外,其余各项观察指标差异均有统计学意义(P<0.05)。结论:护士分层管理下小组包干制整体护理明确各级护士的职责,便于责任落实到人,能明显减少母乳喂养问题的发生,提高纯母乳喂养率。Objective：To evaluate the effect of team overall-rationing system whole nursing under layer management in the obstetrics ward on improving breast-feeding quality.Method：Divide the obstetrics nurses into 3 layers,i.e.team charge-advanced chargenurse-primary chargenurse,carry out team overall-rationing system whole nursing,define their responsibilities in promoting breastfeeding.Compare puerpera＇s tender breast,cracked nipple,nipple illusion incidence and pure breastfeeding rate at leaving hospital,before and after the execution of the method.Result：Before executing the method,the rates of puerpera＇s tender breast,cracked nipple,nipple illusion incidence and pure breastfeeding at leaving hospital were respectively 27.8%,35.5%,3.7% and 68.7%,and 11.8%,16.2%,1.5% and 85.2% after the method execution;except for nipple illusion rate,others had marked difference of statistical meaning.Conclusion：The team overall-rationing system whole nursing under layer management defines the nurses＇ duties,helpful to individual duty execution,can obviously reduce breastfeeding problems and improve pure breastfeeding rate.
