摘  要:目的探讨瑞芬太尼对腹腔镜子宫肌瘤患者围术期脑氧代谢平衡及子宫血流动力学的影响。方法选取2009年8月~2011年11月于本院进行腹腔镜子宫肌瘤手术的86例患者为研究对象,将其随机分为对照组(芬太尼组)43例和观察组(瑞芬太尼组)43例,将两组患者麻醉前及麻醉后10min、20min、30min的脑氧代谢平衡及子宫血流动力学指标进行分析及比较。结果观察组麻醉后10min、20min、30min的脑氧代谢平衡及子宫血流动力学指标的波动幅度均小于对照组,麻醉后不同时段两组各指标比较,P均<0.05,均有显著性差异。结论瑞芬太尼在腹腔镜子宫肌瘤手术中的应用效果要好于芬太尼,其对患者的脑氧代谢平衡及子宫血流动力学的影响更小,安全性也更高。Objective To study the influence of Remifentanil for cerebral oxygen metabolic balance and uterine hemodynamics of patients treated with laparoscopic hysteromyoma surgery during perioperative period.Methods 86 patients treated with laparoscopic hysteromyoma surgery in our hospital from August 2009 to November 2011 were selected as research object,and they were randomly divided into control group(Fentanyl group)43 cases and observation group(Remifentanil group)43 cases,then the indexes of cerebral oxygen metabolic and uterine hemodynamics of two groups before and after the anesthesia at 10min,20min and 30min were analyzed and compared.Results The fluctuation range of cerebral oxygen metabolic and uterine hemodynamics of observation group were all smaller than those of control group,all indexes of two groups at different times after the anesthesia were compared,all P〈0.05,there were all significant differences.Conclusion The application effect of Remifentanil for laparoscopic hysteromyoma surgery is better than that of Fentanyl,the influence for cerebral oxygen metabolic balance and uterine hemodynamics of patients is smaller,the security is higher.
