氨氯地平联合黄芪注射液对肾功能不全者造影剂相关性肾病的影响

摘　　要：目的:探讨氨氯地平与中药黄芪联用对肾功能不全患者造影剂相关性肾病的影响。方法:116例轻度肾功能不全患者被随机分为:无用药对照组28例,氨氯地平组30例,黄芪组28例,氨氯地平和黄芪联用组30例;在冠脉造影前预防性应用氨氯地平及黄芪注射液,检测应用造影剂前后血尿素氮(BUN)、肌酐(Scr)、尿β2微球蛋白(β2-MG),并计算肌酐清除率(Ccr)。结果:(1)与造影前比较,应用造影剂后无用药对照组患者Ccr水平[(64.1±9.5)ml/min]明显下降,BUN、尿β2-MG水平明显升高(P<0.05～<0.01);(2)造影后与无用药对照组比较,氨氯地平组、黄芪组以及联合用药组Ccr[(49.9±12.6)ml/min比(60.4±11.2)ml/min比(60.5±12.3)ml/min比(68.3±10.6)ml/min]显著上升,且联合用药组的明显高于两单用药组(P<0.05～<0.01),两单用药组间无明显差异(P>0.05);BUN、尿β2-MG水平氨氯地平组、黄芪组的明显高于无用药对照组(P<0.05～<0.01),联合用药组与无用药对照组无显著差异(P>0.05)。结论:造影剂碘普罗胺可引起肾功能不全患者肾功能恶化,氨氯地平与黄芪联用对肾功能有较好的保护作用。Objective： To study influence of amlodipine combined with traditional Chinese medicine Astragalus membranaceus（AM）on nephropathy induced by radiocontrast media in patients with mild kidney insufficiency.Methods：A total of 116 patients with mild kidney insufficiency were randomly divided into no-drug control group（n=28,no received drug trentment）,amlodipine group（n=30）,AM group（n=28） and amlodipine ＋ AM group（n=30）.The amlodipine and/or AM injection were received before coronary angiography,levels of blood urea nitrogen（BUN）,urine β2-microglobulin（β2-MG） and creatinine clearance rate（Ccr） were measured in all groups except no-drug group before and after coronary angiography（CAG）.Results：（1）Compared with before CAG,there was significant decrease in Ccr [（64.1±9.5） ml/min vs.（49.9±12.6） ml/min],significant increase in levels of blood BUN and urine β2-MG in no-drug control group after CAG,P0.05~0.01;（2）Compared with no-drug control group after CAG,Ccr [（49.9±12.6） ml/min vs.（60.4±11.2） ml/min vs.（60.5±12.3） ml/min vs.（68.3±10.6） ml/min] significantly increased in amlodipine group,AM group and amlodipine＋AM group,and that of amlodipine＋AM group was significantly higher than that of amlodipine group and AM group（P0.05~0.01）;Compared with no-drug control group the levels of BUN,urine β2-MG of amlodipine＋AM group were similar（P0.05）.Conclusion： Contrast medium iopromide may aggravate kidney in sufficiency in patients with kidney insufficiency,but amlodipine combine AM possess better protection for kidney function.
