慢性胃炎患儿血中胃泌素、胃动素的变化及关系的研究

摘　　要：目的　探讨慢性胃炎患儿血中胃肠激素的变化及其相互间的关系。方法　应用放射免疫测定法 (RIA)、聚合酶链反应 (PCR)等技术对 2 0例经纤维胃镜和病理证实为慢性胃炎患儿血中胃泌素 (GS)、胃动素 (MTL)质量浓度进行测定 ,并以 30例正常儿童作对照。对慢性胃炎组患儿取空腹胃液行幽门螺杆菌 (HP) -DNA检测。结果　慢性胃炎组血清GS及血浆MTL质量浓度均明显高于正常对照组 (P <0 0 1)。两种胃肠激素质量浓度间无相关关系 (r =0 0 5 2 5 ,P >0 0 5 ) ;GS质量浓度增高与胃液 pH值无相关关系 (r=- 0 0 2 85 ,P >0 0 5 ) ;2 0例慢性胃炎患儿胃液HP -DNA阳性 15例。结论　慢性胃炎患儿某些胃肠激素的分泌异常可能直接或间接参与了慢性胃炎的发病过程。HP感染可能是导致胃肠激素分泌异常的另一重要因素。儿童高胃泌素血症可能是综合因素共同作用的结果。推测GS。Objective To study alteration of blood gastrointestinal hormone and their relationship in chronic gastritis children.Methods Concentration of gastrin and motilin were determinated by technical of radioimmunoassay to 20 chronic gastritis children who diagnosiced by fibergastroscope and pathology,and 30 normal children were compared.To assess DNA of helicobacter pylori in gastritic juice of 20 cases chronic gastritis children by using polymerase chain reaction.Results Gastrin concentration of chronic gastrit is group was significant higher than that of control group(P<0 01),and so did concentration of motilin.There were no reference between these two gastrointestinal hormones(r=0 0525,P>0 05);there were no reference between increase of gastrin concentration and pH of gastritis juice(r=-0 0285,P>0 05);there were 15 HP-DNA positive cases in 20 chronic gastritis children.Conclusion There were parasecretion of some gastrointestinal hormones in chronic gastritis children,and it direct or indirect effected the proceed of chronic gastritic; infection of helicobacter pylori was an important factor which caused gastrointestinal hormones parasecretion, hypergastrinemia of children caused rather by many facters than pH of gastric juice;there probably were not cooperation or antangonism between the secretion and release proceed of gastrin and motilin.
