摘 要:目的:利用结核病地理信息系统为社区卫生服务设计标准化的结核病网络管理程序,实现社区结核病管理规范化。方法:应用地理信息和网络技术,按结核病防治管理规范建立信息系统,实施社区网络联动的结核病重点人群预警、转诊、追踪、督导服药、访视、密切接触者筛查、质量控制和分析评估。结果:信息系统2009-2011年中发出社区结核病预警1 148起,重点人群预警1 114起,发现聚集性事件4起;社区卫生服务中心报告疑似肺结核1 613例,转诊率为99.81%,辖区内患者追踪到位率91.88%;发现涂阳肺结核患者5 559例,筛查密切接触者21 217例,筛查率为98.74%,筛查发现有肺结核可疑症状者236例,患者62例;2008-2010年新发涂阳肺结核患者5 020例,治愈率为91.89%,系统管理率为98.90%。社区信息质量控制结果通过软件生成数据报表和图表。结论:结核病地理信息网络系统应用对社区结核病规范化、智能化和科学化的实时管理提供了有效的支持。Objective To design standard network management program for community health services based on Geographic Information System(GIS),achieving standardization of community tuberculosis management.Methods The information system was established under the direction of Standardization in TB Prevention and Management,using geographic information(GIS) and network technology.The procedure of TB management including focus group warning,referring,tracing,supervising of medication,visiting,screening of close contactors,quality controlling,analyzing and evaluating were all interactive via internet among communities in Hangzhou.Results A total of 1 148 warnings in community and 1 114 warnings among focus group were sent out by GIS between 2009 and 2011,and 4 clustered TB epidemic were found.1 613 suspected cases were reported by community health services,with the total referral rate 99.81%,the total tracing in place rate of suspected cases 91.88%.5 559 smear-positive pulmonary tuberculosis patients were diagnosed and 21217(98.74%) close contactors were screened,in which 236 patients with suspicious symptoms and 62 diagnosed patients were found.91.98% of the total 5020 smear-positive pulmonary tuberculosis patients,who were diagnosed between 2008 and 2010,were cured,and the systematic management rate was 98.90%.All the results of quality controlling were reported by the information system in the form of tables and graphs.Conclusion The GIS offers effective support for establishing standard,intelligent and scientific real-time TB management at community level.
