浆膜腔积液细胞块免疫化学染色鉴别增生的间皮细胞与腺癌

摘　　要：目的探讨几项细胞块免疫化学染色鉴别浆膜腔积液中增生的间皮细胞和腺癌细胞。方法收集腺癌、间皮增生的浆膜腔积液66例,制成细胞块切片,进行HE及癌胚抗原(CEA)、钙视膜蛋白(Calretinin)、波形蛋白(vimen-tin)免疫化学染色。结果普通涂片发现浆膜腔积液中转移性腺癌46例,间皮细胞增生20例,CEA在46例转移性腺癌中,32例阳性,14例阴性,阳性率69.57%;20例增生的间皮细胞中2例阳性(均为弱阳性),18例阴性,阳性率10.00%。46例转移性腺癌中calretinin 1例阳性,45例阴性,阳性率2.17%;20例增生的间皮细胞17例阳性,3例阴性,阳性率85.00%。46例转移性腺癌中vimentin阳性表达2例,44例阴性,阳性率4.35%;20例增生的间皮细胞中18例阳性,2例阴性,阳性率90.00%,差异有统计学意义(P<0.01)。结论细胞块免疫化学染色对鉴别浆膜腔积液中增生的间皮细胞及腺癌细胞有重要参考价值。Objective To explore immunochemistry staining use for identification mesothelial cells and adenocarcinoma of dropsy of serous cavity.Method Collecting 66 samples of adenocarcinoma or dropsy of serous cavity which including proliferous mesothelial cells,all of those made into cell block sections,and then carried out HE and carcinoembryonic antigen（CEA）,calretinin,vimentin immunochemistry dyeing.Results In dropsy of serous cavity,46 metastatic adenocarcinoma were found by traditional smears,20 cases were mesothelial cell proliferation.CEA in 46 patients with metastatic adenocarcinoma,32 cases were positive,14 cases were negative,the positive rate was 69.57%.20 cases of proliferous mesothelial cells,2 were positive,18 cases were negative,the positive rate was 10.00%.Among 46 cases of metastatic adenocarcinoma,1 case was calretinin positive,45 cases were negative,the positive rate was 2.17%.Among 20 cases of hyperplasia mesothelial cells,17 cases were positive,3 cases were negative,the positive rate was 85%.In 46 cases of metastatic adenocarcinoma,2 cases were vimentin positive,44 cases were negative,the positive rate was 4.35%,in hyperplasia mesothelial cells,18 were positive,2 cases were negative,the positive rate was 90%,the different had statistical significance,P0.01.Conclusions Cell block immunochemistry staining had important reference value on identification mesothelial cells and adenocarcinoma cells of dropsy of serous cavity.
