摘  要:目的:研究兔单次照射急性放射性心脏损伤(RIHD)的病理组织学改变。方法:新西兰大白兔23只,制定三维治疗计划后利用直线加速器行单次照射。依据照射剂量分为A组(临床相关剂量17只)和B组(高剂量6只),A组剂量分别为0、10、14、18 Gy,动物分别为1、3、6、7只;B组剂量分别为22、30、40、54 Gy,动物分别为3、1、1、1只。随访4个月后进行病理学检查,并对两组动物RIHD的严重程度进行比较分析。结果:A和B组RIHD的严重程度评分平均值分别为1和1.5。急性RIHD表现主要是心包异常,严重时可出现心包填塞,心肌和瓣膜损伤相对较轻。结论:心脏在接受单次大剂量照射后,发生急性RIHD较为多见,且与心脏受照射总剂量密切相关,剂量越大,发生RIHD几率越高,越严重。Objective: To analyze the pathologic features of acute radiation induced heart disease(RIHD) after a single dose radiation in a Newzealand rabbit model.Methods: According to the radiation dose,23 rabbits were divided into group A(clinical related-dose group) and group B(high dose group),then each group was divided into several subgroups according to the dose.In group A,the subgroup's dose and the number was 0,10,14,18 Gy and 1,3,6,7 respectively.In group B,the dose and the number was 22,30,40,54 Gy and 3,1,1,1 respectively.Each rabbit was radiated in a single dose model.After that,each was followed up for 4 months and then the pathologic features were analysed.Results: The average grade of the severity of RIHD of group A and group B was 1 and 1.5 respectively.Moreover,acute RIHD mainly manifested as pericardial abnormality and pericardial tamponade could happen with high dose.In contrast,myocardium and valvular abnormalities were relatively uncommon.Conclusion: Large single dose radiation can lead to severe acute RIHD and the severity may be related to the radiation dose.
