山东莱洲湾湿地长角血蜱无形体16SrRNA检测分析

摘　　要：目的调查山东莱洲湾湿地优势硬蜱——长角血蜱(Haemaphysalis longicornis)人粒细胞无形体病原携带及分子流行病学特征。方法 2010年5月至10月,对莱洲市第一人民医院ICU及感染科收治的26名无形体确诊病例及6名可疑病例家居环境采集游离蜱114只,家畜山羊、牛及狗体表寄生硬蜱456只,分类鉴定后分组研磨提取DNA,巢氏PCR扩增无形体16SrRNA基因并测序及序列分析。结果 69组样本PCR扩增阳性率为43.5%。序列进化分析显示无形体病原主要分3个基因群,T1优势群占检出序列的60%,该群与我国安徽无形体院内感染患者及山东沂源地区发热患者检出序列归为一类。这一群在我国周边国家日本、韩国及德国野生动物及媒介蜱中均有检出。结论东北亚环西太平洋鸟类迁徒的重要"中转站"及越冬、栖息和繁殖地—莱洲湾湿地长角血蜱存在较高无形体病原体携带率,加强当地临床发热病人无形体鉴别诊断并采取必要防控措施具有重要临床及公共卫生意义。In order to investigate the prevalence and genetic diversity of A.phagocytophilum in H.longicornis in Laizhou Bay areas,Shandong province.Based on the 26 confirmed patients with anaplasmoses and 6 probable patients hospitalized in Laizhou people＇s hospital during May Oct,2010,a total of 456 H.longicornis were collected from domestic animals including goats,cattle and dogs and 114 ticks were collected from wild fields around patients＇ house.A.phagocytophilum 16SrRNA genes were amplified and analyzed from ticks＇ samples by nested PCR.The prevalence of A.phagocytophilum in ticks was 43.5% and 3 genetic groups of A.phagocytophilum were identified.The T1 dominant group account up 60% identified sequences and was closely related with the sequences identified in patients in the outbreak of nosocomial transmission of anaplasmoses in Anhui province in 2006.Here we concluded that a high positive rates of A.phagocytophilum was observed in H.longicornis in Laizhou Bay areas and differential diagnoses of anaplasomoses in clinics should be emphasized.
