强心合剂对前临床心力衰竭患者血流动力学及血浆N-末端脑钠肽前体水平的影响

摘　　要：目的观察强心合剂对前临床心力衰竭患者血流动力学及血浆N-末端脑钠肽前体(NT-pro-BNP)水平的影响。方法 60例前临床心力衰竭患者随机分为西药基础治疗组(对照组)和强心合剂治疗组(治疗组),观察2组临床疗效、射血分数、血浆NT-pro-BNP水平和无创血流动力学指标的变化。结果治疗后治疗组肾虚症状积分明显低于对照组,心排血量、每搏量、射血分数显著高于对照组,胸部液体含量、外周血管阻力、血浆NT-pro-BNP水平显著低于对照组。结论强心合剂可能通过加强前临床心力衰竭患者心脏的收缩功能、改善血流动力学(增加心排血量,提高射血分数)及调节神经体液因子(降低血浆NT-pro-BNP浓度),干预心室重构发生、发展,从而延缓心力衰竭的进程。Objective It is to observe the clinical effect of Qiangxin mixture on Hemodynamics and plasma NT-pro-BNP in patients with pre-clinical heart failure.Methods 60 cases of patients with pre-clinical heart failure were randomly divided into Qiangxin mixture treatment group（treatment group） and basic western medicine treatment group（control group）.The clinical curative effect,and the changes of ejection fraction,plasma NT-pro-BNP level and noninvasive hemodynamic monitoring indexes were observed in both groups.Results After treatment,the integral of the symptoms of renal asthenia in treatment group was obviously lower than that in control group.however,cardiac output,stroke volume,ejection fraction were significantly higher than that in control group,thoracic liquid content,systemic vascular resistance and plasma NT-pro-BNP lever were lower than that of control group.Conclusion Qiangxin mixture can inhibit the generation and progress of left ventricular remodeling by enhancing cardiac contraction,improving hemodynamics（increasing cardiac output and ejection fraction）and regulating the neuroendocrine factors（lowering the level of plasma NT-pro-BNP）,so that it could postpone the processes of heart failure.
