摘  要:目的观察凋膜止崩方对无排卵型功能失调性子宫出血(ADUB)大鼠模型子宫内膜细胞凋亡及凋亡基因bcl-2、bax表达的影响,探讨凋膜止崩方治疗ADUB子宫内膜增生症的机制。方法采用去势大鼠雌激素负荷法建立ADUB模型,随机分成空白组、模型组、实验小剂量组和实验大剂量组共4组。造模成功后第3天,宫腔注药处死,采用TUNEL法检测细胞凋亡,免疫组化法检测bcl-2、bax基因的表达。结果 (1)模型对照组与空白组相比,凋亡指数增高(P<0.05);大、小剂量组与模型组相比,凋亡指数均明显升高(均P<0.05);大、小剂量组间差异无统计学意义(P>0.05)。(2)模型组与空白组相比,bcl-2表达明显升高,而bax表达降低,bcl-2/bax比值显著升高(均P<0.05)。(3)实验大、小剂量组与模型组相比,bcl-2表达明显降低,bax表达明显升高,bcl-2/bax比值显著下降(均P<0.05)。结论凋膜止崩方能够降低bcl-2 mRNA的表达,而增强bax mRNA的表达,通过下调bcl-2/bax比值,诱导过度增生的子宫内膜细胞凋亡可能是其发挥祛除子宫内膜作用的分子机制。Objective To explore the role and the possible mechanism of Diaomo Zhibengfang on the rats with endometria hyperplasia through observing the influence of the Diaomo Zhibengfang on apopto- sis of endometrial cells and expression of bcl-2 and bax in the rats with ADUB. Methods Model of ADUB were induced by injection of oestradiol benzoate (0.15ml/100g) into castrated female healthy a- dult SD rats. Rats were randomly divided into four groups: control group, model group, big and small dose of Diaomo Zhibengfang. The model of ADUB was builded. After three days the rats in all groups were sacrificed. The expression levels of bcl-2 and bax were checked by TUNEL and immunohistochemis- try. Results ( 1 ) Compared with control group, apoptotic index increased in the rats of the model group ( P 〈 O. 05 ). The apoptotic indexes of small and big dose of Diaomo Zhibengfang groups were higher than the model group ( P 〈 0.05 ). There is no difference between small and big dose of Diaomo zhibengfang groups (P 〉 0.05 ). (2) The expression of bcl-2 increased, and the expression of bax decreased distinct- ly, bcl-2/bax ratio increased distinctly in the rats of model group ( P 〈 0.05 ) ; (3) Compared with model group, the expression of bcl-2 was decreased distinctly of big and small dose groups, the expression of baxwas increased, bcl-2/hax ratio decreased distinctly (P 〈 0.05). Conclusion Diaomo Zhibengfang plays a role in the endometrial hyperplasia by accelerating the ecclasis of uterine endometrial. One of the mech- anisms maybe related to inhibiting the expression of bcl-2 mRNA, enhancing the expression of bax mR- NA,and decreasing bcl-2/bax ratio.
