自行脱落胆道支架在腹腔镜胆总管切开联合胆道镜探查术中的应用研究

摘　　要：目的:探讨自行脱落胆道支架在腹腔镜胆总管切开联合胆道镜探查术中的临床应用价值。方法:回顾性分析我院2009年7月至2011年7月收治的胆囊结石合并胆总管结石患者63例,其中支架引流33例,T管引流30例,比较两组患者手术时间、术中出血量、肛门排气时间、术后住院时间、结石残留率、置管时间、胆漏以及肝功能等。结果:两组患者术后肛门恢复排气时间、胆漏发生率、结石残留率及术后1周肝功能比较,差异均无统计学意义(P>0.05),而支架组患者手术时间、术中出血量、术后住院时间和置管时间均少于T管组,具有统计学差异(P<0.05)。结论:自行脱落胆道支架应用于腹腔镜胆总管切开联合胆道镜探查术中,减少住院时间及置管时间,安全可靠,促进患者康复,值得临床推广应用。Objective： To explore the value of biliary-stent to fall off in laparoscopic common bile duct biliary stent in laparoscopic surgery in the joint exploration cholangioscopy clinical. Method： A retrospective analysis of our hospital from Jul. 2009 to Jul. 201 l merger of gallbladder stones were treated 63 patients with ＇common bile duct stones, of which 33 cases of stent drainage, T-tube drainage of 30 patients, two groups were compared operative time, blood loss volume, anal exhaust time, postoperative hospital stay, residual stone rate, catheter time, bile leakage and liver functions. Result： Postoperative recovery anal exhaust time, the incidence of bile leakage, residual stone rate and liver function after 1 week, the ditterence was not sta- tistically significant { P〉 0.05 ）, while the stent group were operative time, intraoperative in blood loss, postoperative hospital stay and catheter time were less than T-tube group, with a statistically significant difference （ P 〈0.05）. Conclusion： Biliary stents used to fall off laparoscopic corer on bile duct exploration intraoperative cholangioscopy combined to reduce hospital stay and catheter time, safe and reliable, and promote the rehabilitation of patients, is worthy of clinical application.
