摘 要:通过对上矢状窦64层螺旋CT平扫状态下的定量研究,为脑静脉血栓形成的诊断提供依据。方法:回顾性分析60例患者的64层螺旋CT平扫图像和血常规数据。将上矢状窦分为上、中、下3段,比较分析徒手(根据上矢状窦横断面形状选择兴趣区)和圆形(中间、前方、左侧、右侧)兴趣区法测量的各段窦腔内CT值,分析其与血红蛋白含量之间的相关性。结果:上矢状窦各段CT值差异无统计学意义,徒手和圆形(中间)兴趣区法测得的窦腔内CT值差异无统计学意义。各种圆形兴趣区法测得的上矢状窦CT值差异具有统计学意义。徒手和圆形(中间)兴趣区法测得的上矢状窦CT值与血红蛋白含量呈线性正相关。结论:徒手兴趣区法是定量观测上矢状窦的良好手段,通过血红蛋白含量可以推算出窦腔内CT值正常范围,出现异常时,提示腔内有血栓形成。Objective: To assess the superior sagittal sinus (SSS) quantitatively with unenhanced 64 detector row helical CT (UECT), in order to provide a basis for the imaging diagnosis of cerebral venous thrombosis (CVT). Methods: A total of 60 patients were examined in this study. The data were obtained from adult patients undergoing both UECT head examination and complete blood count within 24 h of the scan. The SSS was divided into the upper, middle and lower segments. The CT attenuation values were measured from each patient using freehand and circle (middle, anterior, left and right) region of interest (ROD techniques. Statistical analysis were preformed to assess the correlation between the hemoglobin levels and CT attenuation values. Results: There were no statistical differences between the CT attenuation values obtained from the different segments of the SSS by using freehand and circle ROI. There were statistical differences between the CT attenuation values by using different types of circle ROI. There was statistically significant correlation between the hemoglobin levels and the CT attenuation values by using freehand and circle ROI. Conclusion: The freehand ROI is the best way to assess SSS quantitatively with UECT. The HU of SSS could be calculated by the hemoglobin level, and quantitative assessment of SSS may prove useful in the diagnosis of the cerebral venous thrombosis.
