摘　　要：目的探讨关节镜下外侧支持带松解治疗髌骨外侧高压综合征的效果和临床意义。方法本组25例中,男2例,女23例;单膝20例,双膝5例,共30膝;平均年龄50.1岁(35～71岁)。结果术后所有病例均得到随访,平均随访7个月(3～23个月),疗效评定参照改良膝关节Lysholm评分标准。95分以上为优,85～94分为良,65～84分为可,65分以下为差,本组25例30膝中,优22膝,良6膝,可2膝。Lysholm评分从术前平均(61.03±6.02)分(49～75分)提高到术后平均(93.14±3.64)分(86～99分),经配对t检验,差异有统计学意义,症状明显改善(t=6.28,﹤0.001)。术后2膝有血肿形成,占6.7%,均系术中为达到充分松解而切断部分股外侧肌。结论关节镜下外侧支持带松解是一种微创的软组织平衡手术,能够有效地缓解髌骨外侧高压综合征引起的髌股关节疼痛,且并发症较少。OBJECTIVE To discuss the effective and clinical value of arthroscopic lateral retinaculum release treatment in excessive lateral pressure syndrome.METHODS 25 patients were with the average age of 50.1 years old（35-71 years）,included 2 males and 23 females,20 cases of single keens and 5 cases of double keens.RESULTS All patients were followed up after an average duration of 7 months（3-23 months）.Improved reference standard of Lysholm score was used to evaluate the treatment efficacy：95 points or more for excellent,85-94 points for good,65-84 points for satisfactory,and 65 points or less for the poor.25 patients with 30 keens included the excellent in 22 keens,good in 6 keens,satisfied in 22 keens,good in 6 keens,satisfied in 2 knees.The mean preoperative Lysholm score（61.03 ± 6.02）points（49 to 75 points）increased to postoperative average（93.14 ± 3.64）points（86-99 points）.Significant differences could be found before and after operation（t = 6.28,P﹤0.001）.There were 2 knees of hematoma formation after operation,accounting for 6.7%.Those ascribed that surgery did not achieve the full release and cut off part of the vastus lateralis muscle.CONCLUSION Arthroscopic lateral retinaculum release treatment is a minimally invasive surgery of the soft tissue balance,which could effectively alleviate the patellofemoral pain and fewer complications caused by excessive lateral pressure syndrome.
