鼻内镜下鼓膜穿刺结合咽鼓管用药治疗分泌性中耳炎64例临床研究

摘　　要：目的探讨鼻内镜下鼓膜穿刺结合咽鼓管用药治疗分泌性中耳炎的临床疗效。方法将某院64例患者随机分为两组,对照组:30例患者(36耳)采用鼓膜穿刺加药物治疗(对照组);观察组:34例患者(36耳)在对照组的治疗的同时,采用鼻内镜下咽鼓管吹张治疗。统计分析两组患者的疗效和并发症情况。结果观察组治疗痊愈24耳,有效9耳,无效2耳,总有效率为94.44%;对照组治疗痊愈19耳,有效9耳,无效8耳,总有效率为77.78%。两组的治疗总有效率比较差异有统计学意义(P﹤0.05)。所有患者术后随访6～12个月,未见1例有并发症。结论鼻内镜下鼓膜穿刺结合咽鼓管用药治疗分泌性中耳炎,操作简单方便,治愈率高,无并发症发生,可以作为治疗分泌性中耳炎的一种有效的方法推广应用。OBJECTIVE To investigate the efficacy of tympanic membrane puncture under nasal endoscope with medication in eustachian tube in treatment of secretory otitis media.METHODS 64 patients in our hospital were randomly divided into two groups,control group：30 patients（36 ears）using tympanic membrane puncture plus medical therapy（control group）;observation group：34 patients（36 ears）in the treatment group at the same time,the use of endoscopic treatment of eustachian tube blowing sheets.Statistically analysed efficacy and complications of patients in the two groups.RESULTS The treatment cured 24 ears,effective 9 ears,invalid 2 ears,the total effective rate was 94.44%;control group cured 19 ears,effective 9 ears,invalid 8 ears,the total effective rate was 77.78%.The total effective rate in two groups was significantly different（P﹤0.05）.All patients were followed up for 6 to 12 months,1 patient had no complications.CONCLUSION Tympanic membrane puncture under nasal endoscope with medication in eustachian tube in treatment of secretory otitis media is a simple operation,with high cure rate and no complications,which can be used as an effective way and its application should be promoted.
