摘  要:目的探讨Kv1.3在持续不卧床腹膜透析(continuous ambulatory peritoneal dialysis,CAPD)相关性腹膜炎患者腹腔巨噬细胞中的表达及低钾对巨噬细胞功能的影响。方法以2009年10月至2010年8月在江西省人民医院住院的CAPD相关性腹膜炎患者30例作为实验组,以2009年10月至2010年8月在江西省人民医院住院的CAPD无腹膜炎患者30例作为对照组。流式细胞仪检测腹腔巨噬细胞Kv1.3表达;按不同K+浓度分为4组:5.4mmol.L-1 K+组(正常浓度对照组)、2.0mmol.L-1 K+组、4.0mmol.L-1 K+组和10.0mmol.L-1 K+组。将Raw 264.7巨噬细胞与不同K+浓度组置于RPMI1640培养基中共同培养。经LDH、中性红吞噬、大肠杆菌(E.coli)菌落形成单位(colony forming unit,CFU)测定等,观察其对巨噬细胞活性、吞噬能力、杀菌能力的影响。结果与对照组比较,实验组患者透出液中巨噬细胞Kv1.3表达下降,但差异无统计学意义[平均荧光强度:(8.78±2.30)%vs(9.54±3.61)%,P>0.05]。2.0、4.0、10.0mmol.L-1 K+组Raw264.7细胞培养4、8、12、24hLDH水平与5.4mmol.L-1 K+组比较差异均有统计学意义(均P<0.05),2.0、4.0、10.0mmol.L-1 K+组Raw264.7细胞培养8、12、24hLDH水平与细胞培养4h比较差异均有统计学意义(均P<0.05)。与5.4mmol.L-1 K+组比较,2.0mmol.L-1 K+组吞噬中性红显著下降(0.21±0.02vs 0.15±0.01,P<0.01),4.0、10.0mmol.L-1 K+组吞噬中性红均无明显变化(0.21±0.02vs 0.19±0.03,0.22±0.05,均P>0.05);与4.0mmol.L-1 K+组比较,2.0mmol.L-1 K+组吞噬中性红显著下降(0.19±0.03vs 0.15±0.01,P<0.01);与10.0mmol.L-1 K+组比较,2.0、4.0mmol.L-1 K+组吞噬中性红显著下降(0.22±0.05vs 0.15±0.01,0.19±0.03,P<0.01)。与5.4mmol.L-1 K+组比较,2.0mmol.L-1 K+组吞噬细菌数及杀菌数显著下降(P<0.05);与10.0mmol.L-1K+组比较,2.0、4.0mmol.L-1 K+组吞噬细菌数及杀菌数均显著下降(均P<0.01)。结论 CAPD相关性腹膜炎巨噬细胞Kv1.3表达无显著的改变,但低钾能显著地降低巨噬细胞吞噬Objective To explore the expression of Kv1.3 in celiac macrophages and the influence of low potassium on macrophage function in continuous ambulatory peritoneal dialysis(CAPD)-related peritonitis.Methods Thirty patients with CAPD-related peritonitis hospitalized in Jiangxi province people's hospital between October 2009 and August 2010 were selected as experimental group,and 30 CAPD patients without peritonitis as control group.The expression of Kv1.3 in celiac macrophages was detected by flow cytometry.RAW 264.7 macrophages were cultured in RPMI1640 medium supplemented with different concentrations of K+(5.4,2.0,4.0 and 10.0 mmol·L-1K+).The LDH,neutral red phagocytosis and E.coli colony forming units(CFU) were determined to observe the effects of low K+ on macrophage activity,phagocytic capacity and bactericidal ability.Results The expression of Kv1.3 in effusion macrophages in experimental group was lower than that in control group,but the difference was not statistically significant[mean fluorescence intensity:(8.78±2.30) % vs(9.54 ± 3.61)%,P0.05].The levels of LDH in 5.4 mmol·L-1K+ treatment group were significantly different from those in 2.0,4.0 or 10.0 mmol·L-1K+ treatment group after culture for 4,8,12 and 24 hours(all P0.05).Furthermore,LDH levels after culture for 4 hours were obviously different from those after culture for 8,12 or 24 hours in 2.0,4.0 and 10.0 mmol·L-1K+ treatment groups(all P0.05).Compared with 5.4 mmol·L-1K+,neutral red phagocytosis was significantly decreased by 2.0 mmol·L-1 K+ treatment(0.21±0.02 vs 0.15±0.01,P0.01),but was not changed by 4 and 10.0 mmol·L-1K+ treatment(0.21±0.02 vs 0.19±0.03 and 0.22±0.05,all P0.05).Compared with 4.0 mmol·L-1K+,neutral red phagocytosis was significantly decreased by 2.0 mmol·L-1K+ treatment(0.19±0.03 vs 0.15±0.01,P0.01).Compared with 10.0 mmol·L-1K+ treatment,neutral red phagocytosis was significantly decreased by 2.0 and 4.0 mmol·L-1K+ treatment(0.22±0.05
