塞来昔布、氨酚羟考酮和加巴喷丁用于腹腔镜手术术后镇痛的比较研究

摘　　要：目的比较塞来昔布、氨酚羟考酮与加巴喷丁用于腹腔镜手术术后镇痛与镇静的效果。方法选取行腹腔镜胆囊切除术患者60例，随机分为3组：塞来昔布组（A组），氨酚羟考酮组（B组）和加巴喷丁组（C组），每组20例。予以术前、术后口服给药，观察患者术后疼痛评分（VAS）、镇静评分（Ramsay）和不良反应。结果3组静态VAS评分在清醒后0．5h与术后6、12、24、48h时差异无统计学意义（P〉0．05）；3组动态VAS评分在术后6、12、24h时比较有统计学差异（P〈0．05），其中C组最高，A组次之，B组最低；镇静评分除术后48h时外，其余各时点3组比较有统计学差异（P〈0．05），其中C组最高，B组次之，A组最低；3组不良反应发生率无统计学差异（P〉0．05）。结论塞来昔布、氨酚羟考酮和加巴喷丁均可安全有效地用于腹腔镜手术术后镇痛；三者镇痛效果比较为氨酚羟考酮最强，镇静效果加巴喷丁最好。Objective To investigate the postoperative analgesia effects of oral administration of celecoxib, oxycodone / acetaminophen and gabapentin on patients with laparoscopic cholecystectomy. Methods Sixty patients undergoing laparoscopie cholecysteetomy were randomly divided into three groups., celecoxib group （group A）, oxycodone / acetaminophen group （group B） and gabapentin group （group C）, 20 cases in each. The medicines were given at the time before and after surgery. VAS, Ramsay score and side effects were observed after the surgery. Results No significant difference was found in VAS at rest between the three groups （P〉0.05）. VAS in moving at 6,12, 24 h after the surgery was significantly different among the groups （P〉0.05）, in which VAS in group C was the highest, second was in group A, the lowest was in group B. There was a significant difference in Ramsay scores at 0. 5,6,12,24 h after the surgery among the groups （P〉0.05）. Ramsay score in group C was the highest, second was in group B, the lowest was in group A. There was no significant difference in the side effects among the groups （P〉0.05）. Conclusion All the methods of oral administration of celecoxib, oxycodone / acetaminophen and gabapentin are effective in perioperative analgesia after laparoscopic cholecystectomy. The analgesic effect of oxycodone / acetaminophen is the strongest. The sedative effect of gabapentin is the best.
