地佐辛预先镇痛用于人工流产术的临床效果

摘　　要：目的比较地佐辛与芬太尼预先镇痛复合丙泊酚静脉麻醉在人工流产术中的效果及不良反应。方法拟行人工流产术的孕妇90例，随机分为3组，每组30例。D组预先静脉注射地佐辛0．1mg／kg，F组预先静脉注射芬太尼1μg／kg，P组为对照组。各组均静脉注射丙泊酚2mg／kg。观察各组呼吸循环变化，记录丙泊酚总用量、麻醉诱导时间、唤醒时间、宫缩痛及不良反应。结果D组和F组术中血压、心率、丙泊酚总用量、丙泊酚注射痛和体动发生率较P组降低，F组术中呼吸抑制发生率高于其余2组（P〈0．05）。结论地佐辛预先镇痛复合丙泊酚麻醉用于人工流产术的效果优于芬太尼复合丙泊酚麻醉或单纯丙泊酚麻醉，且无芬太尼的呼吸抑制作用。Objective To observe and compare the effects and adverse reactions of dezocine with fentanyl in preemptive analgesia combined with propofol intravenous anesthesia on artificial abortion. Methods Ninety pregnant women scheduled for abortion were randomly divided into three groups, 30 cases in eactx. Dezocine 0.1 mg/kg was given intravenously in group D, fentanyl 1 μg/kg in group F, nothing in group P. Propofol 2 mg/kg was given intravenously in all the groups. The changes in respiratory and circulatory systems were observed. The total amount of propofol, induction time, wake-up time, uterine contraction, pain and adverse reactions were recorded. Results Compared with group P, HR, MAP, propofol consumption, the incidences of injection pain and body movement during the operation were decreased in group D. The rate of respiratory inhibition was higher in group F than that in the others. Conclusion Dezocine preemptive analgesia combined with propofol intravenous anesthesia has a better effect than preemptive fentanyl combined with propofol anesthesia or propofol anesthesia alone during artificial abortion, and has no respiratory inhibition.
