摘  要:目的探讨颜面播散性粟粒状狼疮(1upus miliaris faciei,LMF)的发病机理。方法选择2001年1月-2009年12月本科门诊20例颜面播散性粟粒状狼疮患者和15例银屑病患者皮损及10例色素痣边缘正常皮肤的石蜡切片,用免疫组化法观察LL-37,HBD-2和HBD-3的表达。结果与正常组皮肤比较.LL-37,HBD-2和HBD-3在LMF的表皮全层高表达,以中上层为著(P均〈0.001);在毛囊、皮脂腺、小汗腺、血管壁、组织细胞和巨细胞的胞浆亦有高表达;它们在银屑病皮损中为中表达和高表达:但是HBD.2和HBD.3在LMF皮损中的表达强度显著高于银屑病皮损(P均〈0.001).LL-37的表达则与银屑病相似(P〉O.05)。结论抗茵肽LL-37,HBD-2和HBD-3均参与LMF的免疫反应过程。Objective To explore the possible role of antimicrobial peptides (LL-37, HBD-2 and HBD-3) in the pathogenesis of Lupus miliaris faciei. Methods Tissue specimens collected from outpatients were used to detect the expression of LL-37, HBD-2 and HBD-3 from 20 patients with lupus miliaris faciei, 15 pa-tients with psoriasis, and 10 with normal skins at the edge of pigmented nevus from Jan.2001 to Dec. 2009 in our hospital. Results LL-37, HBD-2 and HBD-3 were detected mainly in the upper layer of epidermis, appendicle, vascular endothelial cells and langhans cells in the dermis of LMF lesions. Their expressionswere much higher than in that of normal skin (P all 〈 0.001 ). The expression of LL-37 was similar of that in the psoriatic lesions(P 〉 0.05).However, the expressions of HBD-2 and HBD-3 were much higher than that of the psoriatic lesions (P all 〈 0.001 ). Conclusion LL-37, HBD-2 and HBD-3 have all participated inthe process of immune response of Lupus miliaris faciei.
