雾化吸入硫酸特布他林、布地奈德及氨溴索治疗AECOPD疗效观察

摘　　要：目的:探讨雾化吸入硫酸特布他林、布地奈德及氨溴索治疗COPD急性发作(AE-COPD)的疗效。方法:将90例AECOPD患者随机分为治疗组和对照组各45例。对照组采用常规治疗加用硫酸特布他林雾化吸入,治疗组在常规治疗基础上加用硫酸特布他林、布地奈德混悬液及氨溴索联合雾化吸入,其中硫酸特布他林和布地奈德混悬液每次各1 ml,2次/日,经高压氧驱动雾化;氨溴索每次2 ml,2次/日,两组疗程均为10 d。比较两组患者治疗前、后PaO2、PaCO2、SaO2、第一秒用力呼气容积(FEV1)及FEV1/FVC[FEV1与用力肺活量(FVC)的比值],以及两组间治疗后差异。结果:治疗10 d后,两组上述指标治疗后均较治疗前有改善(P<0.05),治疗组疗效较对照组优(P<0.05)。结论:联合吸入硫酸特布他林、布地奈德及氨溴索治疗AECOPD能较好地改善患者的肺功能。Objective：To investigate the nebulized terbutaline sulfate,budesonide and ambroxol hydrochloride in the treatment of acute exacerbation of chronic obstructive pulmonary disease（AECOPD）and clinical application value.Methods：Ninety patients with AECOPD were randomly divided into control group and treatment group with 45 cases in each group.The control group received routine therapy combined with terbutaline sulfate aerosol inhalation;the treatment group on the basis of conventional therapy plus terbutaline sulfate and budesonide suspension and ambroxol hydrochloride oral liquid combined with inhalation,terbutaline sulfate and budesonide suspension,ambroxol hydrochloride oral liquid two times a day after hyperbaric oxygen drive atomization.The treatment duration is 5 to 7 days.Comparison of two groups of patients before treatment,after the arterial partial pressure of oxygen（PaO2）,arterial partial pressure of carbon dioxide（PaCO2）,arterial oxygen saturation（SaO2）,forced expiratory volume in one second（FEV1） and FEV1/FVC[（the ratio of FEV1 and Forced vital capacity（FVC））.Results：After 10 d,all indexes had significant changes（P〈0.05）,and the clinical efficacy of the treatment group is better than that of control group（P〈0.05）.Conclusion：The combination of inhaled terbutaline sulfate,budesonide and ambroxol treatment can preferably improve the lung function in patients with AECOPD.
