空心可吸收螺钉治疗胫骨髁间隆突撕脱骨折临床分析

摘　　要：目的探讨关节镜下空心可吸收螺钉内固定术治疗胫骨髁间隆突撕脱骨折的疗效。方法 2007年3月-2010年10月,收治17例胫骨髁间隆突撕脱骨折患者。男性10例,女性7例;年龄14～43岁,中位年龄26.5岁。致伤原因:交通事故伤9例,运动伤6例,扭伤1例。对17例胫骨髁间隆突撕脱骨折患者进行关节镜下复位,使用空心可吸收螺钉内固定。结果患者术后切口均为Ⅰ期愈合,所有患者获10～16个月(平均12个月)随访。末次随访时,Lach-man试验及轴移试验均呈阴性,Lvsholm膝关节功能评分平均为(93.6±2.5)分。所有患膝恢复健侧活动度,膝关节稳定。结论关节镜下空心可吸收螺钉内固定治疗胫骨髁间隆突撕脱骨折具有创伤小、复位固定可靠、术后康复快及并发症少等优点,有利于早期功能锻炼,同时避免二次手术,可获得满意疗效。Objective To investigate the effectiveness by leverage and fixation using absorbed empty hollow nail to treat tibial intercondylar eminence avulsion fractures under the arthroscope.Methods Between March 2007 and October 2010,17 patients with tibial intercondylar eminence avulsion fractures were treated.There were 10 males and 7 females with a median age of 26.5 years（range,14-43 years）.The injury causes were traffic accident in 9 cases,sport in 6 cases,and spraining in 1 case.17 patients were treated tibial intercondylar eminence avulsion fractures under the arthroscope,using absorbed empty hollow nail internal fixation.Results All incisions healed by first intention.Seventeen patients were followed up 12 months on average（range,10-16 months）.At last follow-up,the results of Lachman and anterior drawer tests were negative in all patients.The Lysholm score was 93.6±2.5.All patients were able to return to their pre-injury activity and daily life,the knee joint was stable.Conclusion Leverage and fixation using absorbed empty hollow nail was minimally invasive,satisfied reduction and fixation,benefit for rehabilitation and with fewer complication in treating the tibial intercondylar eminence avulsion fracture under the arthroscope,so it was benefit for early functional exercises,avoidance reoperative,and can achieve excellent results.
