急诊应用小剂量肾上腺素治疗重度哮喘的疗效观察

摘　　要：目的探讨急诊应用小剂量肾上腺素在重度哮喘治疗中的临床疗效。方法将32例急诊重度哮喘患者随机分为治疗组(肾上腺素组)和对照组(常规治疗组)。对照组(15例)将氨茶碱0.5 g加入500 ml液体中静脉滴注维持,同时静脉注射甲强龙(甲泼尼龙琥珀酸钠)80 mg;治疗组(17例)在对照组的治疗方法上加用1∶1000肾上腺素针剂0.3 mg皮下注射,如果无效,每20 min重复皮下注射肾上腺素0.3 mg,最多不超过3次(肾上腺素总量<1.0 mg)。观察两组患者治疗前后临床疗效和各项指标。结果两组患者治疗后的呼吸频率、心率、血氧饱和度有明显改善(P<0.01),与对照组相比治疗组各项指标改善更加明显(P<0.05),治疗组总有效率高于对照组(P<0.05)。结论急诊应用小剂量肾上腺素能更加快速有效地缓解重度哮喘患者的临床症状。Objective To explore the clinical efficacy of the emergency application of small doses of epinephrine in the treatment of severe asthma.Methods 32 cases of emergency patients with severe asthma were randomly divided into treatment group（epinephrine group） and the control group（conventional therapy group）.The control group（n=15） joined aminophylline 0.5 g intravenous infusion of 500 ml in liquid to maintain,while intravenous methylprednisolone（methylprednisolone sodium succinate） 80 mg;the treatment group（n=17） in the control group therapy combined with 1∶1000 on the treatment of subcutaneous injection of epinephrine injection 0.3 mg,if was invalid,every 20 min repeated subcutaneous injection of epinephrine 0.3 mg,up to 3 times（epinephrine gross1.0 mg）.Clinical efficacy and indicators of two groups of patients before and after treatment were observed.Results After treatment,the heart rate,respiration,blood oxygen saturation of the two groups of patients were obviously improved（P0.01）,compared with the control group,the indicators of the treatment group were improved more obvious（P0.05）,the total effective rate of the treatment group was higher than that of the control group（P0.05）.Conclusion Emergency application of small doses epinephrine can more quickly and effectively relieve the clinical symptoms of the patients with severe asthma.
