改良早期预警评分在院前急救中对患者病情评诂的应用研究

摘　　要：目的:探讨改良早期预警评分(MEWS)方法在院前急救中对患者病情评诂的应用价值。方法:收治院前急救患者2478例,采用现场MEWS方法,分析MEWS不同分数段患者的分布特点、病情危重程度以及院前紧急处理的差异,并追踪患者入院后的去向及病情转归。结果:院前急救患者MEWS以0～4分为主(71.27%),5～9分19.73%,≥10分仅9%。不同分数段患者院前紧急处理的比较差异有统计学意义(P<0.005)。MEWS≥5分与0～4分患者入院后收住专科病房和ICU治疗的比率明显增加,病情危重程度及病死率也明显增加,比较差异有统计学意义(P<0.005)。结论:MEWS方法在院前急救中能早期预警潜在危重患者,MEWS分数段越高,病情越危重,病死率就越高,当MEWS≥5分时,应及早采取防治措施,严密监护,对减少死亡和提高抢救成功率有重要的临床意义。Objective：To discuss the value of Application of modified early warning score（MEWS） method in pre-hospital care on the patient＇s condition assessment.Method：The experiment based on 2478 pre-hospital emergency patients from emergency department of ChaoZhou Centrol Hospital from June 1,2010 to May 31,2011,which were observed and analyzed randomly with the MEWS method.we analysis the characteristics of distirbutions of the MEWS in re-hospital emergency patients and severity of illness,as well as the differences of the pre-hospital emergency treatment,and the follow-up after admission to and prognosis.Results：The patients with MEWS 0～4 accounted for 71.27% and the patients with NEWS of 5～9 patients accounted for 19.73%,while the ones with MEWS more than 10 accounted for 9% only.there was significant difference in pre-hospital emergency treatment among the patients with different NEWS（P0.005）.For the patients with MEWS5 and those with MEWS 0-4,rate of hospitalization and admitted to ICU increased,and the severity and mortality increased significantly too,there were significantly difference among The pre-hospital emergency treatment,the severity and mortality（P0.005）.Conclusion：MEWS can assess the pre-hospital patients＇ conditions and predict the risks.the more MEWS,the more severity and mortality.in fact,MEWS is practical for clinical application.
