摘  要:目的:运用MRI评价退变椎间盘与椎间盘血管化之间的关系。方法:根据椎间盘Pm分级和终板变形对71例376个椎间盘的磁共振平扫和增强扫描进行对照研究。结果:376个椎间盘,220个发生不同程度及不同形态强化,强化率为58.51%。PmⅠ级13个,强化0个;Ⅱ级73个,强化17个,强化率23.29%;Ⅲ级178个,强化107个,强化率60.11%;Ⅳ级93个,强化78个,强化率83.87%;Ⅴ级19个,强化18个,强化率94.74%。终板断裂型164个,强化140个,强化率85.37%;终板完整型212个,强化80个,强化率37.74%。Pm分级和终板形态改变均与椎间盘血管化关系密切,统计学检验,P均<0.005。结论:椎间盘退变、终板断裂与椎间盘血管化关系密切,磁共振增强扫描可直观可靠地诊断退变椎间盘的血管化。Purpose: To evaluate the relationship between the degeneration and vascularization of the intervertebral discs by MRI.Methods: We collected 71 cases,including 44 males and 27 females.All patients underwent plain and enhanced MRI examinations.According to the Pm grade and the deformation of end-plates of the discs,a controlled study was conducted.Results: Three hundred and seventy-six discs were obtained from 71 patients including 22 thoracic discs and 354 lumbar intervertebral discs.A total of 220 intervertebral discs were enhanced,the enhanced rate was 58.51%.Of these discs,13 were Pm grade Ⅰ and none of which was enhanced;73 were grade Ⅱ and 17 of which were enhanced,the enhanced rate was 23.29%;178 were grade Ⅲ and 107 of which were enhanced,the enhanced rate was 60.11%;93 were grade Ⅳ and 7817 of which were enhanced,the enhanced rate was 83.87%;19 were grade Ⅴ and 18 of which were enhanced,the enhanced rate was 94.74%.There were 164 discs with fractured end-plate and 140 of which were enhanced,the enhancement ratio was 85.37%;212 with intact endplates and 80 of which were enhanced,the enhanced rate was 37.74%.Both Pm classification of the intervertebral discs and morphological changes of the endplates were correlated with the disc vascularization,P0.005.Conclusion: The post-enhanced MRI can depict the phenomenon of the vascularization of the degenerated intervertebral discs.The degeneration of disc and the defect of endplate were associated with the vascularization.The vascularization may contribute to the degeneration.
