摘  要:目的 观察异丙酚对家兔全脑缺血 再灌注 (I R)期血清白介素 8(IL 8)的合成和释放的影响 ,并探讨其脑保护机制。方法 利用“六血管”阻断建立全脑缺血模型 ,2 4只家兔随机分为假手术组 (A组 ) ,I R组 (B组 )和异丙酚组 (C组 ) ,每组 8只 ,C组缺血 30min再灌注 4h ,缺血前静注异丙酚 5mg·kg- 1 ,随后静滴 2 0mg·kg- 1 ·h- 1 ,直至实验结束 ,B组为单纯缺血 再灌注组 ,A组仅分离血管不阻断血流。分别在缺血前 15min(Io)及再灌注 30min(R1 )、2h(R2 )和 4h(R3)取颈内静脉血 ,测定血清IL 8浓度 ,实验结束取皮层HE染色 ,光镜观察白细胞浸润及神经元损伤程度。结果 C组R1~R2 时IL 8无明显变化 (P >0 0 5 ) ,仅在R3时较Io和A组有显著升高 (P <0 0 5 ) ,而B组在R1 ~R3时分别增加 2 2 8、2 97、3 74倍 ,同C组和A组相比有显著性差异 (P <0 0 5 ) ;光镜下C组白细胞浸润及神经元损伤程度较B组明显减轻。结论 异丙酚除有抗氧化作用外 ,还能有效抑制IL 8的合成和释放 ,这可能是异丙酚减轻脑I R损伤的又一重要机制。Objective To determine the inhibitory effect of propofol on the production and release of interleukin 8(IL 8) following cerebral ischemia reperfusion(I/R) Methods Twenty four rabbits were divided randomly into three groups: group A without vessels occlusion and propofol administration; group B with cerebral ischemia induced by clipping bilateral external,internal carotid arteries and vertebral arteries for 30min and then declamping with the reperfusion lasting 4 h; group C with propofol administration (5mg·kg -1 ) 10min before the occlusion and followed by intravenous infusion of 20mg·kg -1 ·h -1 till the end of procedures Blood samples were obtained through the catheter, the tip of which was positioned at the jugular bulb, to measure the serum concentrations of IL 8 and malondialdehyde (MDA), the activity of superoxide dismutase (SOD) 10min before the occlusion (I 0), 30min (R 1), 2h (R 2) and 4h (R 3) following reperfusion Hematoxylin and eosin staining for the left temporal cerebral tissue was processed Results Compared with those at I 0 following reperfusion in group A, IL 8 levels remained unchanged significantly at R 1 and R 2 (P>0 05),increased markedly at R 3 in group C (P<0 05) IL 8 levels in group B were higher markedly during reperfusion than those before ischemia and following reperfusion in group A and group B(P<0 05) MDA levels in group C and group A were significantly lower at R 1, R 2 and R 3 than those in group B respectively (P<0 05) But the changes of SOD were contrary to those of MDA in groups B and C Under light microscope, there were no obvious leukocyte aggregation, infiltration,vessel sheath and less karyopyknosis in the group C ,whereas, those changes were more obvious in group B Conclusions In addition to antioxidation, propofol suppresses effectively the production and release of IL 8, to protect the brain from ischemia and reperfusion injury
