摘  要:目的探讨经颈外静脉和股静脉途径建立房间隔缺损动物模型的两种方法。方法选用封闭群滇南小耳猪12头,月龄9~10个月,体重18~25kg。在X线引导下,分别经颈外静脉(6头)和股静脉(6头)途径行房间隔穿刺和球囊扩张术,建立房间隔缺损动物模型,比较两种方法在手术时间和X线暴露时间上的差异。结果 12头除1头死于术中急性心脏压塞,1头死于术后感染外,其余动物均成功建立模型并存活,术后即刻超声心动图显示ASD直径大小为(1.16±0.15)cm,ASD边缘与二尖瓣,三尖瓣,上腔静脉,下腔静脉,右上肺静脉及冠状窦的距离≥5mm,颈外静脉途径建立房间隔缺损模型手术时间(35.00±5.72)min和X线暴露时间(4.16±0.48)min与股静脉途径建模的手术时间(48.40±4.72)min和X线暴露时间(8.60±0.30)min两者比较,颈外静脉建立模型时间短于股静脉建立模型时间(t=5.15,P<0.05)。结论经颈外静脉行房间隔穿刺术及球囊扩张术制作猪房间隔缺损动物模型是一种有效,简便,微创,可重复的方法,在手术时间和X线暴露时间方面优于股静脉建立模型方式。Objective To establish atrial septal defect (ASD) model in pigs via right external jugular vein (EJV) and femoral vein and to assess the defference in the two approaches. Methods Twelve pigs of the Diannan small ear species were selected with an average age of 9 to 10 months,weighting 18 - 25 kg. Under X-ray exposure,ASD model of pigs via right external jugular vein ( n = 6) and femoral vein ( n = 6) by atrial septal puncture and balloon dilation were established comparisons in X-ray exposure time 2 opention time was made. Results One pig died of acute cardiac tamponade,and anoher one died of postoperative infection. The achievement of ASD were showed (1. 16 ± 0. 15) cm under echocardiography immediately after operation. The distance from the rim of ASD to MV,TV,IVC,SVC,PV,CS was all over 5 mm. The operation time (35. 00 ± 5. 72) min and X-ray exposure time (4. 16 ± 0. 48) min via right EJV was shorter compared with the operation time (48. 40 ± 4. 72) min and X-ray exposure time (8. 60 ± 0. 30) min via femoral vein (all P 0. 05) . Conclusions To establish a ASD model in pigs via right external jugular vein is an effective,simple,minimally invasive and repeatable method. The method via EJV is superior to via femoral vien in the operation time and X-ray exposure time.
