原发性高血压患者瘦素及脂蛋白水平的临床观察

摘　　要：目的通过检测原发性高血压(EH)患者血清瘦素(Leptin)、脂蛋白水平变化,探讨血清瘦素与脂蛋白的相关性及其临床价值。方法 EH患者80例(分为EHⅠ期和EHⅡ期)和对照组30例分别测定外周血瘦素、高密度脂蛋白胆固醇(HDL-C)及低密度脂蛋白胆固醇(LDL-C)水平,分析血清瘦素水平与HDL-C、LDL-C的关系。结果 EH患者血清瘦素及LDL-C水平均高于对照组(P<0.05),HDL-C水平低于对照组(P<0.05);EHⅡ期与EHⅠ期比较,血清瘦素及LDL-C水平均增高(P<0.05),HDL-C水平降低(P<0.05)。EH患者外周血瘦素水平与HDL-C水平呈负相关(r=-0.307、P<0.05),与LDL-C水平呈正相关(r=0.516,P<0.05)。结论 EH患者体内存在瘦素和HDL-C、LDL-C的异常表达,三者变化与血压分级有关,EH患者瘦素水平与脂蛋白具有相关性。Objective By detecting serum leptin and lipoprotein levels in essential hypertension（EH） patients,and to explore the relevance and clinical value of serum leptin and lipoprotein.Methods 80 EH patients（divided into EHⅠ period and EHⅡperiod） and 30 controls were measured serum leptin,high density lipoprotein-cholesterol（HDL-C） and low density lipoprotein-cholesterol（LDL-C） levels from blood samples and analysis of the relationship between the serum leptin levels and HDL-C,LDL-C.Results The levels of serum leptin and LDL-C in EH patients were significantly higher than the NC group（P0.05）,while HDL-C levels were significantly lower than the NC group（P0.05）;Serum leptin and LDL-C levels were significantly higher（P0.05） in the EHⅠperiod patients in comparison with EHⅡperiod patients,and HDL-C levels were significantly lower（P0.05） than EHⅡperiod patients.In the EH patients,leptin level in peripheral blood negatively correlate with HDL-C levels（r =-0.307,P0.05）,and positively correlate with LDL-C level（r= 0.516,P 0.05）.Conclusion This study demonstrates that the abnormal expression of Leptin and HDL-C,LDL-C in EH patients are relationship with blood pressure levels,and leptin levels of EH patients are correlated with the lipoprotein.
